Transrektal ultrasonografi eşliğinde uygulanan prostat iğne biyopsisinde lokal anestezi: Lidokain - prilokain (EMLA) krem uygulama süresi ağrıyı azaltmada önemli mi?
Amaç: Transrektal ultrasonografi eşliğinde yapılan prostat iğne biyopsilerinde ağrıyı azaltmak için kullanılan lidokain-prilokain (Emla %5) krem uygulandıktan sonra geçen sürenin ağrı üzerine etkisi olup olmadığını araştırdık. Gereç ve yöntem: Prostat spesifik antijen düzeyi >4ng/ml ya da parmakla rektal muayenede nodül saptanan ardışık 76 hasta çalışmaya alındı ve bu hastalar randomize olarak iki gruba ayrıldı. Grup I’de 34 hastaya Emla %5 krem uygulandıktan 15 dk sonra grup II’de 42 hastaya ilaç uygulamasından 45 dk sonra transrektal ultrasonografi eşliğinde prostat iğne biyopsisi yapıldı. İşlem sonrasında 10 nokta aralıklı linear vizüel ağrı skalası (VAS) hastalara sorularak kaydedildi. Tüm hastalar işlemden 1 gün önce başlamak ve sonrasında 2 gün kullanmak üzere siprofloksasin 500 mg günde iki kez kullandı ve hepsine 12 kadran prostat biyopsisi uygulandı. Sonuçlar istatistiksel olarak bağımsız t testi ve Mann-Whitney U testi ile değerlendirildi. Bulgular: Hastaların ortalama yaşları, ortalama PSA düzeyleri, ortalama prostat volümleri ve ortalama ağrı skorları (VAS skoru) arasında istatistiksel olarak anlamlı farklılık saptanmadı. Ortalama VAS ağrı skoru grup I’de 2.40±1.13, grup II’de ise 2.32±1.88 olarak kaydedildi. Grup II’deki hastalarda ortalama VAS skorları Grup I’den daha düşük olmasına rağmen her iki grup arasında istatistiksel olarak anlamlı farklılık saptanmadı. Sonuç: Transrektal ultrasonografi eşliğinde uygulanan prostat biyopsilerinde ağrıyı azaltmak için ilaç uygulandıktan biyopsi işlemi yapılıncaya kadar geçen sürenin 15 dk’dan daha uzun olması önemli olmamaktadır.
Local anesthesia in transrectal ultrasound - guided prostate biopsy: Is important duration of lidocaine - prilocaine (EMLA 5%) administration for relief of the pain
Objective: The aim of our study was to evaluate the efficacy of the duration of lidocaine-prilocaine (Emla 5%) administration for relief of the pain during transrectal ultrasound-guided prostate biopsy. Materials and methods: Seventy-six male consecutive patients with prostate-specific antigen (PSA) levels >4ng/ml or who had prostate nodules on digital rectal examination were randomly divided into two groups. In Group I (n=34), Emla 5% cream was performed 15 min before the biopsy. In Group 2 (n=42), the same drug was performed 45 min before the biopsy. A 10-point linear visual analogue score (VAS) was used to record the pain experienced during biopsy procedure. All patients commenced a 2-day course of oral antibiotic prophylaxis 1 day prior to the procedure: ciprofloxacin 500 mg twice a day and underwent 12 core TRUS guided prostate biopsy. Results were statistically analyzed by Mann-Whitney U test. Results: No significant difference was found the mean age, mean PSA level, mean prostate volume and mean VAS pain scores in the two groups. The mean VAS scores were 2.40±1.13 in Group I and 2.32±1.88 in Group II. Although the mean VAS scores were lower in Group II, there was no statistical difference between groups. Conclusion: Long duration of the lidocaine-prilocaine (Emla 5%) administration is not important during the transrectal ultrasound-guided prostate biopsy. Fifteen minutes is comfortable before the procedure.
