Hipodermokliz (Sürekli subkütanöz infüzyon): Onkoloji hastalarında etkili bir hidrasyon tedavisi (?)

Hipodermokliz (HDC) özellikle uzun süreli bakım gören hastalarda, intravenöz kateterizasyonun sağlanamadığı durumlarda başvurulabilecek güvenli, uygulanması kolay, hasta uyumunun yüksek olduğu bir yöntemdir. HDC subkütanöz yolla sıvı infüzyonunu sağlamada kullanılan bir yoldur. Klinik uygulamalarda hafif ve orta dereceli dehidratasyonun tedavisinde, subkütan yolla uygulanabilecek antibiyotik, morfin, karbamazepin gibi ilaçların replasmanında kullanılmaktadır. Yöntemin kolay uygulanabilir olması, hastanın hidrasyon ve ilaç veriminin kendi evinde veya bakım evinde devam etmesine olanak sağlamaktadır. Bu durum göz önünde bulundurulduğunda, HDC özellikle yaşlı populasyondaki mortalite ve morbidite ile yakından ilişkili olan dehidratasyonun önlenmesinde, intravenöz kateterizasyona güvenli bir alternatif oluşturmaktadır. Bu derlemede son yıllarda eski güncelliğini tekrar kazanmış olan HDC yönteminin tekniğini, endike ve kontrendike olduğu durumları, avantaj ve dezavantajlarını, istenmeyen etkilerini, HDC `nin hyaluranik asit ile birlikte uygulanmasını ve HDC’nin özellikle terminal dönemdeki hastalar açısından onkolojideki kullanım yerini açıklamayı amaçladık.

Hypodermoclysis (Continous subcutaneous infusion): An efficient hydration treatment for oncology patients (?)

Hypodermoclysis (HDC) is safe, easy to use, and comfortable way to deliver medication for the patients who have chronic illness and couldn’t use intravenous line. HDC can supply some intravenous hydration with subcutaneous route. In clinical practice, DHC can be used in the treatment of mild and moderate dehydration. Morphine, some kind of antibiotics, and carbamazepine can be applied via DHC safely. Moreover it can be performed easily; most of the medication and hydration treatment can be given in home care or nursing care settings. After all, HDC is a very safe method to prevent dehydration that can cause severe morbidity especially for older patients and it may be used as alternative method to intravenous catheterization for these purposes. Herein, we aim to discuss indication and contraindications, advantages and disadvantages, complications, application with hyaluronic acid of HDC and also its general usage for terminally ill cancer patients
