Torakotomi ile sonuçlanan yabancı cisim aspirasyonları
Amaç: Trakeobronşiyal yabancı cisim aspirasyonları, her yaş grubunda görülebilir. Bazen yıllarca gözden kaçıp tanısı gecikebilir. Gelişmiş tedavi tekniklerinin kullanıma girmesine rağmen, hayatı tehdit edebilen bir sağlık problemidir. Tanısal gecikmeye ve farkında olmamaya bağlı olarak, herhangi bir yabancı cisim akciğerde uzun süre kalırsa bronşektazi, obstrüktif amfizem, tekrarlayan pnömoni, akciğer apsesi ve bronkoplevral fistül gelişebilir. Bu tür komplikasyonlara neden olmaması için veya sebep olduğu komplikasyonu tedavi etmek amacıyla torokotomi uyguladığımız hastaları ve sonuçlarını sunmayı amaçladık. Yöntem ve Gereçler: Ocak 2005–Aralık 2011 tarihleri arasında yabancı cisim aspirasyonu şikayeti ve olası aspirasyonla ilişkili akciğerde kronik dönemde gelişen irreversibl değişiklere bağlı non spesifik şikayetler ile başvuran 12 hasta retrospektif olarak değerlendirildi. Uygulanan bronkoskopik yöntemlerle hiçbir şekilde mevcut yabancı cisim çıkarılamadı ve sonrasında yabancı cismi çıkartma amaçlı olarak tüm hastalara torakotomi uygulandı. Bulgular: Olguların % 67’si (8/12) kadın , % 33 (4/12) erkek olup ortalama yaş 26,43 idi (14-61 yaş). Aspire edilen yabancı cisimler; dokuz olguda iğne, bir olguda pisi pisi otu, bir olguda kalem başlığı ve bir olguda diş olarak değerlendirildi. Sonuç: Trakeobronşiyal yabancı cisim aspirasyonu şüphesi olan tüm olgulara bronkoskopi uygulanmalıdır. Aspire edilen yabancı cismin bronş sisteminin distalinde olduğu ve rijid bronkoskopinin yetersiz olduğu durumlarda hastalarda torakotomi uygulanmalıdır.
Foreign body aspirations resulted with thoracotomy
Aim: Tracheobronchial foreign body aspirations, can be seen in all age groups. Sometimes the diagnosis can be missed for years. Despite the advanced treatement techniques, they are still life-threatening health problem. Depending on the diagnostic delay and not to be aware of, if any foreign body remains in the lung for a long time, can cause bronchiectasis, obstructive emphysema, recurrent pneumonia, lung abscess and bronchopleural fistula. We aimed to submit the patients whom we applied thorocotomy to avoid such of these complications or to treat this type of complication caused by them. Material And Methods: Between January 2005-December 2011 12 patients who admitted with the complaints of foreign body aspiration and non-specific symptoms associated with irreversible chronic changes in the lungs due to possible foreign body aspirations were evaluated retrospectively. The current foreign bodies could not be removed in any way with the bronchoscopic techniques and all patients underwent thorocotomy for the purpose of removing foreign bodies. Results : 67 % of the cases (8/12) women and 33 % (4/12) were male and mean age was 26.43 (14-61 years). Aspirated foreign bodies; needle in 9 cases, infloresence in 1 case, pencap in 1 case and tooth in 1 case. Conclusion: Bronchoscopy should be applied to all cases suspected tracheobronchial foreign body aspiration. If the aspirated foreign body is in distal localization of the bronchial system and rigid bronchoscopy is unsuccessful then thorocotomy must be applied to the all patients.
