Sigara ve alkol kullanımı ile İ.V. tek doz propofolün serum lidokain düzeyleri üzerine etkisi

Amaç: Lidokain anestezi uygulamalarında endotrakeal entubasyonla oluşabilen sempatomimetik yanıtları önlemek amacıyla oldukça yaygın olarak kullanılan bir lokal anestezik ve antiaritmik ajandır. Lidokain karaciğerde başkca CYP1A2 ve CYP3A4 izoenzimleri ile yıkılmaktadır. Aynı izoenzimler üzerinde etkili ajanlardan biri olan propofol CYP3A4 inhibisyoııu yaparken, alkol CYP2E1 ve CYP3A4, sigara ise CYP1A2 ve CYP2E1 izoen-zimlerini indüklemektedir. Bu çalışmada i.v. tek doz propofol uygulamasının ve kronik sigara-alkol kullanımının i.v. bolus olarak verilen lidokainin kan serum düzeyleri üzerine etkilerini incelemeyi amaçladık. Gereç ve yöntem: ASA I-II grubu 26 erişkin hasta, sigara-alkol kullanım öykülerine ve kullanılan indüksiyon ajanına göre dört grupta değerlendirildiler. 1. grup kronik sigara kullanan hastalar, 2.grup kronik sigara ve alkol kullanan hastalar, 3. ve 4. grup ise sigara ve alkol kullanmayan hastalardan oluşmaktaydı. 1., 2. ve 3. grupta, anestezi indüksiyonu için propofol kullanılırken 4. grupta (kontrol grubu) anestezi indüksiyonunda tiyopental kullanıldı. Anestezi indüksiyonundan hemen önce tüm hastalara l mg/kg intravenöz lidokain uygulandı. Lidokain düzeyleri ölçümü için 1., 5., 10., 20., 40. ve 60. dakikalarda 1'er ml arteriyel kan örnekleri alındı. Bulgular: Gruplar arasında serum lidokain doz zaman eğrilerinde ve eğri altında kalan alanlarda [EAA (0-60) $mu$g/ml /dak] istatistiksel olarak anlamlı fark izlenmedi (p>0,05). Ölçülen zamanlarda serum lidokain konsantrasyonları da gruplar arasında istatistiksel olarak anlamlı farklılık göstermedi. Sonuç: Bu araştırmada, kronik sigara-alkol kullanımının ve bu hastalara uygulanan tek doz iv. bolus propofolün, serum lidokain düzeylerini anlamlı biçimde etkileme-diği sonucuna varılmıştır.

The effects of single I.V. dose propofol and cigarette-alcohol consuming on serum lidocaine levels

Objective: Lidocaine is a widely used local anesthetic and antiarrhythmic agent to prevent sympathomimetic reactions associated with endothraceal eııtubation in general anesthesia. Lidocaine is mainly metabolized by cytochrome CYP1A2 and CYP3A4 isoenzymes in the liver. Propofol inhibits CYP3A4, whereas ethanol CYP2E1, CYP3A4 and tobacco induces CYP2E1, CYP1A2 isoenzymes mainly. In this study our aim was to evaluate the effect of propofol (single i.v. dose) and chronic cigarette - alcohol consumption on serum lidocaine concentrations. Material and method: Twenty six ASA I-II adult patients undergoing elective surgery were enrolled in the study. The patients were assigned to one of four study groups. Group l were chronic smokers, Group 2 were both alcohol and cigarette consumers, Groups 3 and 4 consisted of non-smokers and non-alcoholics. Group 1— 3 received propofol while Group 4 (the control group) received thiopental for the induction of anesthesia. Before the induction of general anesthesia, all patients received Img/ kg 2 % lidocaine hydrochloride i.v bolus. Arterial blood samples were drawn at 1, 5,10, 20, 40, and 60 minutes. Results: There were no significant differences in the area under the curves (AUC (60)$mu$g/ml/min) between the groups. Similarly there was no significant difference in the measured levels of lidocaine at any time point between the groups (p>0.05). Conclusion: In conclusion, it has been found that the administration of a single dose of propofol and chronic cigarette-alcohol consumption have no effect on serum lidocaine concentrations.
