Beyin ölümü kavramı ve DEÜTF anestezi yoğun bakım ünitesindeki olguların retrospektif incelenmesi
Amaç: Son yıllarda resusitasyon tekniklerinin gelişmesi, suni yaşam desteği ve yoğun bakım ünitelerinin yaygın kullanımı beyin ölümü kavramını gündeme getirmiştir. Beyin ölümü, geri dönüşümsüz komadaki kalbi çalışmaya devam eden olgular için kullanılan bir kavramdır. Beyin ölümü kriterleri; bilincin tam kaybı, Glasgow Koma Skalası'na göre skorun üç olması, spontan solunumun olmaması ve beyin sapı reflekslerinin tamamen kaybını içerir. ABD'de referans olarak alınmış bazı hastanelerde nörologlar yılda 25-30 olguya beyin ölümü tanısı koymaktadırlar. Gereç ve Yöntem: Bu çalışmada, beyin ölümü kavramı; tarihçesi, klinik kriterleri, tanı testleri açısından değerlendirilmiş ve 1997 Ocak-2001 Mart tarihleri arasında Dokuz Eylül Üniversitesi Tıp Fakültesi Anesteziyoloji ve Yoğun Bakım Ünitesindeki beyin ölümü olgusu kayıtları retrospektif olarak incelenmiştir. Bulgular: Bu araştırma sonucunda, dördünün organ bağışı yaptığı belirlenen yedi beyin ölümü olgusu saptanmıştır. Sonuç: Yapılan araştırmada sadece yedi olgu saptanmış olması kayıtların azlığına dikkat çekmektedir.
Brain death consept and the retrospective study of the cases in intensive care unit of the Anesthesiology Department at Dokuz Eylül University Medical Faculty
Objective: in recent years, the advancements in resuscitation techniques and the wide- spread usage of life support and intensive çare units has brought the concept of brain death into attention. Brain death is a concept used only for those cases having a functioning heart in an kreversible stage of coma. Brain death criteria include loss of consciousness, having a score three at the Glasgow Coma Scale, absence of spontaneous respkation and the complete loss of brain stem functions. in USA, in large referral hospitals, neurologists make the diagnosis of brain death for 25 to 30 patients a year. Material and Method: in this study, concepts, clinical criteria, history and confirmational tests of brain death were considered with a retrospective evaluation of brain death case records between January 1997 and March 2001 at Anaesthesiology and Reanimation, intensive Çare Department of Dokuz Eylül University the Faculty of Medicine.Result: As a consequence of this research, the hospital recorded 7 brain death cases of which four had göne organ transplantation. Conclusion: Due to the little considerarion of the subject ör kek of records in hospitals.
