Solunum yolu infeksiyonları ile üriner sistem infeksiyonlarının birlikteliği
Amaç: Solunum yolu infeksiyonu düşündüren müphem semptomlar ile çocuk polikliniğine başvuran ve ateşin nedeni tam açıklanamayan hastalarda üriner sistem infeksiyonu sıklığını belirlemek. Gereç ve Yöntem: Çalışma ateş, öksürük ve burun akıntısı gibi solunum yolu semptomları ile polikliniklere başvuran 268 çocukta (yaş aralığı: 2 ay-16 yaş) gerçekleştirilmiştir. Solunum yolu infeksiyonu bulguları olan bu çocuklara, olası bir üriner sistem infeksiyonu birlikteliğini araştırmak amacıyla tam idrar tahlili ve idrar kültürü yapılmıştır. Kronik hastalığı, üriner sistem anomalileri, diğer konjenital malformasyonları olanlar ve polikliniğe başvurmadan önce antibiyotik kullananlar çalışma dışı bırakılmıştır. Bulgular: 268 hastanın 41’inde (% 15.2) piyüri, 23’ünde de (% 8.5) idrar yolu infeksiyonu tespit edilmiştir. Piyüri saptanan 41 hastanın yalnızca 6’sında (% 14.6) idrar yolu infeksiyonu saptanırken, idrar kültüründe üreme olan 23 hastanın ise yalnızca 4’ünde (% 17.3) piyüri vardı. İdrar kültürlerinde en sık izole edilen patojen Escherichia coli (% 60.8)’ydi ve en yüksek oranda ilaç direnci ampisilin ve trimetoprim-sulfametoksazole karşı saptandı. The coexistence of respiratory system infections and urinary tract infections
Aim: To find out the frequency of urinary tract infections in patients admitted to outpatient clinics with vague symptoms pointing out to respiratory system infections but the origin of the fever is not exactly determined. Material and Method: The study was conducted among 268 (aged between 2 months-16 years) children admitted to outpatient clinics with respiratory system symptoms like fever, cough and rhinorrhea. All these patients representing respiratory tract symptoms had urinalysis and urine cultures in order to search for the coincidence of urinary tract infections. Children having chronic diseases, abnormalities of urinary systems and other congenital malformations and who have used antibiotics prior to coming outpatient clinics were excluded. Results: Pyuria was detected in 41 (15.2 %), urinary tract infection was detected in 23 (8.58 %) of 268 children. Among 41 patients with pyuria, urinary tract infection was determined in 6 (14.6 %) and in 23 children with positive urine culture only 4 (17.3 %) had pyuria. The most common pathogen was Escherichia coli (% 60.8) and ampicillin and trimetoprim-sulphametoxazole had the highest antibiotic resistance rates. Fever was significantly higher in patients with urinary tract infections than children without urinary tract infections (p<0.01). There was no statistically significant difference between other symptoms. The incidence of urinary tract infection was higher in infants aged 2 months-2 years than other age groups (p<0.05). Conclusion: In febrile infants aged 2 months-2 years, attention must be paid to the presence of urinary tract infections. It must be remembered that urinary tract infections can coexist with respiratory system infections, so, urine cultures must be done in this age group if fever accompanies respiratory system symptoms
