Akut ve kronik immün trombositopenik purpura ayırımı tanı anında yapılabilir mi?: Trombosit volüm parametrelerinin önemi

Amaç: İmmun trombositopenik purpura (İTP) trombosit sayısı yanında, volüm parametreleri üzerinde de değişikliklere yol açan klinik bir tablodur. Bu çalışmada akut ve kronik İTP hastalarında trombosit volüm parametrelerinin (ortalama trombosit volümü [MPV], ortalama trombosit dağılım aralığı [PDW]) değerlendirilmesi ve ilk başvuru esnasında bu iki tanının ayırımının yapılabilirliğinin belirlenmesi amaçlandı. Yöntem: Beş yıllık sürede kliniğimizde izlenen 56 (% 80)'sı akut, 14 (% 20)'ü kronik İTP tanılı toplam 70 hastanın tanı anındaki değerleri retrospektif olarak incelendi. Bulgular: MPV değeri akut İTP grubunda 7.0±2.0 fl, kronik İTP grubunda 8.4±0.9 fl olarak saptandı. İki grup arasında istatistiksel anlamlı fark vardı (p0.05). Sonuç: İTP tanısında, trombosit volüm parametrelerinden özellikle MPV değeri hastalığın akut ya da kronik seyredeceği konusunda ön bilgi verebilir. Ancak daha fazla çalışmaya ihtiyaç vardır.

Is it possible to differentiate acute and chronic immune thrombocytopenic purpura at initial diagnosis: Importance of platelet volume parameters

Aim: Immune thrombocytopenic purpura (ITP) cause to changes on the volume parameters of as well as thrombocyte numbers. We wanted to determine the thrombocyte volume parameter values (mean thrombocyte volume [MPV], platelet distribution width [PDW]) in thrombocyter acute and chronic ITP patients and we wanted to find out whether it was possible to differentiate these two at initial diagnostic stage. Method: The diagnostic stage values of 56 (80 %) acute, 14 (20 %) chronic, a total of 70 ITP patients who have been evaluated retrospectively for five years in our clinic. Results: The MPV value was determined as 7.0±2.0 fl in the acute ITP group, as 8.4±0.9 fl in the chronic ITP group. There was a statistical difference between the two groups (p<0.001). The PDW value was 13.1±3.6 fl in the acute group, whereas in the chronic group it was 15.2±3.1 fl. There was not a statistical difference between the two groups (p>0.05). Conclusion: If especially MPV values in the thrombocyte volume parameters are examined in the diagnosis of ITP, it can be determined that the disease is either acute or chronic. But more studies are required in this aspect.
