Postoperatif bulantı-kusmanın önlenmesinde metoklopramid ve dimenhidrinat'ın etkilerinin karşılaştırılması

Tiroidektomi girişimi geçirecek hastalarda postoperatif bulantı-kusma (POBK)'yı önlemede dimenhidrinat'in etkinliğini diğer bir antiemetik ajan olan metoklopramid ile karşılaştırmayı amaçladık. Çalışmamızda; ASA I-II grubundan 40 hasta 20'şer kişilik iki gruba ayrıldı. Metoklopramid grubuna (Grup M) indüksiyondan önce 10 mg metoklopramid iv ve Dimenhidrinat grubuna (Grup D) ise indüksiyondan önce 50 mg dimenhidrinat iv olarak yapıldı. Gruplar arasında bulantı-kusma skoru açısından istatistiksel anlamlılık bulunmadı. Bulantı-kusma görülme sıklığı M grubunda D grubuna göre fazla idi, fakat istatistiksel fark belirlenmedi. Ek antiemetik gereksinimi D grubunda M grubuna göre azdı ve istatistiksel olarak anlamlı değildi. Analjezi gereksinimi ve yan etki açısından gruplar arası fark yoktu. Çalışmamız; elektif tiroidektomi cerrahisi geçiren hastalarda POBK'nın önlenmesinde dimenhidrinatın metoklopramid kadar etkili olduğunu ve rahatlıkla kullanılabileceğini göstermiştir.

A comparison of the effects of metoclopramide and diymenhidraton on postoperative nausea and vomiting

We compared the antiemetic effect of dimenhydrinate with Metoclopramide in patients who had undergone tyroidectomy. 40 patients were divided into two groups with 20 patients in each group. Group M (metoclopramide) received iv 10 mg and Group D (dimenhydrinate) received iv 50 mg dimenhidrat before induction. The nausea-vomiting score was not statistically different between the study groups. The incidence of nausea and vomiting in group M was found to be more frequent in comparison with group D, although it was not statistically significant. The need for additional antiemetics was less in Group D compared to Group M, but this was not statistically significant. There was no difference between the study groups regarding analgesic requirements and side effects. This study has shown that dimenhydrinate is as effective as metoclopramide for the prevention of postoperative nausea and vomiting in elective thyroidectomy patients.
