Türkiye'de spinal anestezi uygulama profili
Bu anket çalışmasında Türkiye'de spinal anestezi yöntemlerinin kullanılma oranları, spinal anestezide teknik ve ilaç tercihiyle ilgili eğilimlerin belirlenmesi amaçlandı. Tek doz spinal anestezi, kombine spinal-epidural anestezi ve kontinü spinal anestezi tekniklerinin kullanımı, anestetik ajanladjuvan ile araç-gereç tercihleri ve komplikasyonları sorgulayan anket hazırlandı. Anketimiz 226 katılımcı tarafından yanıtlandı. Tek doz spinal anestezide en sık kullanılan ajan %0.5 hiperbarik bupivakain (%81) ve tercih edilen iğne Quincke'dir (%75.7). Günübirlik cerrahide spinal anestezi uygulama oranı %73.5 ve tercih edilen ajan %60.2 ile %05 hiperbarik bupivakaindir. Adjuvan ajan kullanım sıklığı %55.3 olup, en çok kullanılan fentanildir (%85.1). Kombine spinal-epidural anestezi %81 gibi yaygın bir kullanım oranına ulaşmıştır. Kateter testinin %49.4 oranında yapılmadığı belirlenmiştir. Kontinü spinal anestezinin kullanımı oldukça az olmasına rağmen (%15.5);%37.1'lik sıklıkla mikrokateter tercih edilmiştir. En sık karşılaşılan spinal anestezi komplikasyonu postdural ponksiyon başağrısı (%50) ve kardiyovasküler arrest (%11.1) olup; ciddi nörolojik komplikasyon 15 katılımcı tarafından bildirilmiştir. Sonuç olarak Türkiye'de bir çok anestezisi spinal anestezi yöntemlerinden bir veya daha fazlasını uygulamaktadır. Aralıklı olarak yapılan anketlerle komplikasyonlarm azalacağını düşünmekteyiz.
