Semptomatik kronik prostatitli hastalarda ejakülasyonun serum prostat spesifik antijen düzeyleri üzerine etkisi

Amaç: Prostat spesifik antijen (PSA) artışına yol açabilen faktörlerden kronik prostatit ve ejekulasyonun etkisi birlikte değerlendirildi. (kronik prostatit olan hastalarda ejakülasyon PSA üzerindeki etkisi olursa daha iyi olur.) Gereç ve yöntem: National Institutes of Health (NIH) prostatit sınıflandırmasına göre kategori II, III-A ve III-B grubunda bulunan 43 hasta ile kontrol grubundaki 12 birey ejekulasyondan hemen önceki ve 30 dakika sonraki serum PSA değerleri ile incelendi. Bulgular: Ejakülasyon öncesi ve sonrası serum PSA değerleri karşılaştırıldığında tüm hasta grupları ve kontrol grubunda ejekulasyonla ortaya çıkan anlamlı bir artış gözlendi. Ejakülasyon ile gözlenen PSA artış oranları tüm hastalarda %84.58, kategori II hastalarda %206.34, kategori III-A hastalarda %26.22 ve kategori III-B hastalarda %32.94, kontrol grubunda da %15.45 olarak bulundu. Kategori II grubunda bulunan hastalardan U.urealyticum ile infekte olanlardaki PSA artış oranı %413,32, S.aureus ile infekte olanlardaki PSA artış oranı ise %32.54 olarak bulundu. Ejakülasyonla ortaya çıkan PSA artış oranlarının karşılaştırılmasında hastalar ve kontrol grubu arasında anlamlı farklılık gözlenmedi. Hasta alt gruplarının kendi aralarında ve kontrol grubu ile karşılaştırılmasında da PSA artış oranlarında istatistiksel anlamlılık gözlenmedi. Bununla birlikte U. urealyticum ile infekte olan hastalarda ejekulasyon ile ortaya çıkan PSA artış oranları dikkat çekici idi Sonuç: Kronik prostatit serum PSA düzeylerini artırabilir, bu artış ejekulasyon sonrası daha belirgindir. Ejekulasyon ile ortaya çıkan artışın özellikle kronik bakteriyel prostatitli hastalarda PSA’nın seminal plasmadan kana geçişinin artmasına bağlı olduğu düşünülebilir.

Effect of ejaculation on prostate specific antigen (PSA) in patients with chronic prostatitis

Objective: To investigate the effect of ejaculation on prostate specific antigen (PSA) in patients with chronic prostatitis. Materials and methods: The study included 43 patients with chronic prostatitis. The patients were classified according to National Institutes of Health (NIH) classification into categories II, III-A and III-B. The control group included 12 patients. PSA was measured just before and half an hour after ejaculation in all patients and control group as well. Results: The increment percentages in serum PSA levels in all patients with ejaculation was 84.58%; in categories II, III-A and III-B were 206.34%, 26.22% and 32.94%, respectively. PSA increment percentage was 412,32% in patients who were in category II, infected with Ureaplasma urealyticum, while in those who were infected with Staphylococcus aureus PSA increment percentage was 32,54%. There was a significant increase in serum PSA levels after ejaculation in all patients and the control group. But there was no significant difference between all patients and the control group in terms of serum PSA increment percentages. Also, there were no significant difference between categories and compared to the control group regarding serum PSA increment percentages. Conclusions: Chronic prostatitis can lead to an increase in serum PSA levels which is particularly significant after ejaculation. It must be emphasized that highest serum PSA increment percentages after ejaculation were detected in patients who were infected with Ureaplasma urealyticum. These findings support the theory that chronic prostatitis patients, increase with ejaculation is due to PSA transfer from seminal plasma to blood.
