Lökositospermi tedavisinin semen parametreleri ve gebelik üzerine olan etkisi

Amaç: Bu çalışmada infertilite yakınmaları nedeniyle kliniğimize başvuran ve yapılan semen analizlerinde lökosi-tospermi tespit edilen olgularda antibiyotik kullanımının semen parametreleri ve gebelik üzerine olan etkilerinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Ocak 2006 ve Aralık 2008 tarihleri arasında infertilite şikayeti ile kliniğimize başvuran 121 erkek olgu çalışmaya alındı. Hastalar semen analizinde lökositospermi tespit edilen 71 olgu (grup 1) ile lökositospermi bulunmayan 50 olgu (grup 2) olmak üzere iki gruba ayrılarak değerlendirildi. Lökositos-permisi olan olgulara toplam 4-6 hafta arasında antibiyotik tedavisi verildi. Olgular öykü, fizik muayene, hormon profili semen incelemesi ve gebelik açısından değerlendirildiler. Bulgular: Lökositospermisi olmayan hastalarla, lökositospermi tespit edilen hastaların tedavi verilmeden önceki semen volümü, sperm sayısı ve sperm hareket yüzdesi açısından karşılaştırıldığında istatistiksel olarak anlamlı bir fark saptanmadı (p>0.05). Lökositospermi tespit edilen hastalarda semen volümü, sperm sayısı ve sperm hareket yüzdesi açısından tedavi öncesi ve sonrası arasında anlamlı bir ilişki saptanmazken lökosit sayısı ile anlamlı bir ilişki saptanmıştır (p=0.043). Sonuç: Çalışmamızda, literatürdeki pek çok araştırmaya benzer şekilde asemptomatik lökositospermi tedavisinin semen parametreleri ve gebelik üzerinde anlamlı düzelmeler sağlamadığı gösterilmiş olup özellikle bu konuda geniş olgu sayısına sahip kanıt düzeyi yüksek randomize, prospektif ve kontrollü çalışmalar yapılmasına gerek bulunduğu söylenebilir.

The effect of leukocytospermia theraphy on semen parameters and pregnancy rates

Objective: In this study, the effect of antibiotic theraphy on semen parameters and pregnancy rates were evaluated in patients with leukocytospermia. In addition, the semen results of patients with leukocytospermia used antibiotic theraphy compared patients without leukocytospermia. Materials and Methods: Between January 2006 and December 2008, to investigate the relationship between leukocytospermia and male fertility, we examined semen specimens from 121 men attending the urology clinic. Patients were classified into two groups, leukocytospermia (n=71) and non-leukocytospermic (n=50). All patients were evaluated past medical history, physical examination, hormone prophile, semen analysis, and pregnancy rates. Patients with leukocytospermia used antibiotic treatment for 4-6 weeks. Results: Our results demonstrated that there were no statistically significant relationship between leukocytospermia and semen parameters compared patients without leukocytospermia. In addition, according to semen parameters we found that the antibiotic theraphy was not effective on semen parameters in patients with leukocytospermia. Sperm motility, volume and concentration were not significantly different in the leukocytospermic group compared to the non-leukocytospermic group (P>0.05). Conclusion: In our study, similarly to the many studies which published in the literature, it has been shown that the treatment of leukocytospermia with antibiotic was not effective on semen parameters and pregnancy rates. However, studies in larger, randomised and controlled series are needed to prove the effect of antibiotic theraphy on semen parameters and pregnancy rates.
