Mesane tümörünün transüretral rezeksiyonunda obturator bloğun yeri

Transüretral cerrahi yüzeyel mesane tümörlerinin ilk basamak tedavisinde altın standart yöntemdir. Mesane boynu, trigon ve lateral duvar yerleşimli tümörlerin rezeksiyonu sırasında obturator sinir stimulasyonuna bağlı uyluk adduktor kaslarında istenmeyen kasılmalar oluşabilir. Mesane duvarının rezeksiyon loopuna doğru ani hareketlenmesi mesane perforasyonuna neden olabilir. Adduktor kaslarda oluşan bu ani spazmı önlemek amacıyla çeşitli yöntemler kullanılmıştır. Bu yazımızda adduktor kas spazmını engellemede etkili bir yöntem olan obturator sinir bloğunun anatomisi, teknik ve sonuçlarını tartıştık.

Obturator nerve block in transurethral resection of bladder tumor

Transurethral surgery is the gold standart first step treatment of non-muscle invasive bladder tumors. During transurethral electroresection in the bladder neck, trigone and lateral wall unintentional contractions of the thigh-adductor muscles may occur due to irritation of the obturator nerve. The sudden displacement of the bladder wall against the cutting loop may cause a perforation of the bladder. To block this reaction varies methods have been described and used. We herein describe the anatomy, technique and discuss the results of local obturator nerve blockade one of the effective option to prevent adductor muscle spasm due to obturator nerve stimulation. .
