Ürolojik rekonstrüktif cerrahide Meckel divertikülü kullanımı
Üriner sistemin rekonstrüksiyonunda gastrointestinal sistem segmentleri yaygın olarak kullanılmaktadır. Meckel divertikülü gastrointestinal sistemin konjenital bir anomalisidir. Meckel divertikülünün ürolojik cerrahide ki kullanımını, kliniğimizde başarı ile gerçekleştirdiğimiz iki üriner diversiyon olgusu ile gözden geçirdik. Meckel divertikülü ile çoğu zaman operasyon sırasında raslantısal olarak karşılaşılmaktadır. Divertikülün mobil olması, kanlanmasının iyi olması, detübülerizasyon gerektirmemesi kullanıldığı anastomoz hattının hem güvenliğini artırmakta hemde cerrahi kolaylık sağlamaktadır. Ayrıca kullanılacak barsak segmenti ihtiyacını kısaltabilmektedir. Ürolojik rekonstrüktif cerrahide barsak segmentleri başarı ile kullanılmaktadır. Operasyon sırasında ortaya çıkan bazı güçlükler nedeniyle kimi zaman başka segmentlere ihtiyaç duyulur. Karşılaşıldığında Meckel divertikülü rekonstrüktif cerrahide kullanılabilecek iyi bir seçenektir.
The use of Meckel’s diverticulum in urological reconstructive surgery
Gastrointestinal segments are commonly used in the reconstruction of urinary tract. Meckel’s diverticulum is a congenital anomaly of gastrointestinal system. In this article, we review the use of Meckel’s diverticulum in urological surgery, and report our two successful urinary diversion cases. Meckel’s diverticulum is usually recognized incidentally during abdominal operations. Since the Meckel’s diverticulum is a structure that is mobilisable, has well blood supply, and does not need to be detubularised, it enhances the safety of anastomosis and provides easiness in surgery. Furthermore, it may also shorten the length of intestinal segment used in surgery. Today, intestinal segments are used in urological reconstructive surgery with great success. However, because of some unexpected difficulties occurred during operation, extra segments may be sometimes necessary. If recognized during surgery, it is favorable to use Meckel’s diverticulum in urological reconstructive surgery.
