ASETABULUM KIRIKLARI CERRAHİ TEDAVİSİNDE POSTERİOR YAKLAŞIM: KLİNİK VE RADYOLOJİK SONUÇLAR

Amaç Asetabulum kırıkları sık rastlanmayan, kompleks ya ralanmalardır. Bu çalışmada posterior yaklaşımlarla opere ettiğimiz asetabulum kırıklı hastaların radyolo jik ve fonksiyonel sonuçlarını değerlendirmeyi amaç ladık. Gereç ve Yöntem 2011 ile 2014 yılları arasında asetabulum kırığı nede niyle opere edilen, en az 1 yıllık takipleri yapılabilmiş olan 36 olgu bu çalışmaya dahil edilmiştir. Retrospek tif olarak planlanan çalışmamızda kırık sınıflandırması Letournel Judet sistemine göre, radyolojik değerlen dirme Matta kriterlerine göre, fonksiyonel skorlar mo difiye Merle d’Aubigne Postel sistemine göre yapıldı. Bulgular Toplam 36 olgunun 25’i sadece posterior yaklaşımla opere edilmişken 11’inde kombine anterior ve poste rior yaklaşım uygulanmıştır. Komplikasyon oranları güncel literatürle uyumlu bulunmuştur. Fonksiyonel skorlar değerlendirildiğinde 29 (%80,6) olguda mü kemmel ve iyi sonuç, 3 (%8,3) olguda orta, 4 (%11,1) olguda kötü sonuç elde edilmiş, radyolojik sonuçlar değerlendirildiğinde 29 (%80,6) olguda mükemmel ve iyi sonuç, 3 (%8,3) olguda orta, 4 (%11,1) olguda kötü sonuç elde edilmiştir. Sonuç Asetabulum kırıkları cerrahisi, teknik olarak zor, komplikasyon oranları yüksek olmasına rağmen dep lase asetabulum kırıklarında açık redüksiyon ve inter nal fiksasyon endike olduğu durumlarda altın standart tedavi yöntemidir. Olgu sayısı kısıtlı olan 36 olguluk serimizde radyolojik ve fonksiyonel sonuçlarımızın başarılı ve güncel literatürle uyumlu olduğunu gördük.

CLINICAL AND RADIOLOGIC OUTCOMES OF OPERATIVE TREATMENT OF ACETABULAR FRACTURES VIA POSTERIOR APPROACH

Objective Acetabular fractures are complex and uncommon injuries. In this study we aimed to evaluate the fun ctional and radiological outcomes of patients with acetabular fractures who were operated via posterior approaches. Materials and Methods Patients with acetabular fractures whom were ope rated between 2011 and 2014 were included in this study. Classification of fractures were done according to Letournel Judet classification. Radiologic outcomes were evaluated according to Matta scores whereas functional outcomes were evaluated according to mo dified Merle d’Aubigne Postel scores. Results Twenty-five of 36 patients were operated via single posterior approach whereas 11 patients were opera ted via both posterior and anterior approaches. Rate of complication is consistent with current articles. Evaluation of functional scores revealed 29 (80.6%) good or excellent results, 3 (8.3%) fair results and 4 (11.1%) poor results. Radiologic outcomes are 29 (80.6%) good or excellent results, 3 (8.3%) fair results and 4 (11.1%) poor results. Conclusions Acetabular surgery is a technically challenging proce dure with high rate of complications. Still it is the gold standard treatment option in displaced fractures of the acetabulum. In our series with 36 patients, our ra diologic and functional outcomes are consistent with current articles.
