STRES ÜRİNER İNKONTİNANSIN TEDAVİSİNDE TENSİON-FREE VAJİNAL TAPE (TVT) VE TRANSOBTURATOR TAPE (TOT) YÖNTEMLERİNİN BAŞARI VE KOMPLİKASYON ORANLARININ KARŞILAŞTIRILMASI

Amaç Tension-free vaginal tape (TVT) ve transobturator tape (TOT) gibi midüretral slingler son yıllarda sıkça kullanılmaktadır. Literatürde değişken komplikasyon ve başarı oranları bildirilmiştir. Biz bu çalışmada, stres üriner inkontinans nedeni ile TVT ve TOT cerrahisi yapılan hastaların komplikasyon ve başarı oranlarını karşılaştırmayı amaçladık. Gereç ve Yöntem Çalışmada, stres üriner inkontinans nedeni (SÜİ) ile TVT ameliyatı yapılan 43 hasta ile TOT ameliyatı ya pılan 38 hastanın verileri retrospektif olarak analiz edildi. Çalışmaya, muayene veya ürodinami ile SÜİ tanısı konan hastalar dahil edildi. Mesane disfonksi yonu yapan nörolojik hastalığı olan, idrar yolu enfeksi yonu, aşırı aktif mesane ve kronik sistiti olan hastalar çalışmaya dahil edilmedi. Hastaların intraoperatif ve postoperatif komplikasyonları ile 6. aydaki cerrahi ba şarı oranları değerlendirildi. Bulgular Hastaların yaş ortalamaları TVT grubunda TOT grubu na göre daha düşük bulunmuştur (sırasıyla 46.2+6.1 ve 53.3+8.1) (p0.05). Operasyon süresi TVT grubun da (19.7+4.3 dakika), TOT grubuna (13.8+2.6 dakika) göre daha uzun bulunmuştur (p0.05). Her iki grupta başarı oranları benzer olarak izlenmiştir ve is tatistiksel fark saptanmamıştır (p>0.05). Sonuç TVT ve TOT operasyonlarının her ikisi de SÜİ teda visinde başarıyla ve güvenle kullanılabilir ve etkinlik açısından aralarında fark yoktur.

COMPARISON OF SUCCESS AND COMPLICATION RATES OF TENSION-FREE VAGINAL TAPE (TVT) AND TRANSOBTURATOR TAPE (TOT) OPERATIONS IN THE TREATMENT OF STRESS URINARY INCONTINENCE

Objective Midurethral slings such as tension-free vaginal tape (TVT) and transobturator tape (TOT) have been used frequently in recent years. Variable complications and success rates have been reported in the literature. In this study, we aimed to compare the complication and success rates of patients undergoing TVT and TOT surgery for stress urinary incontinence. Materials and Methods The data of 43 patients who underwent TVT opera tion and 38 patients who underwent TOT operation for stress urinary incontinence (SUI) were analyzed retrospectively. Patients diagnosed with SUI by phy sical examination or urodynamic were included in the study. Patients with neurogenic bladder dysfunction, urinary tract infection, overactive bladder and chronic cystitis were excluded. Intraoperative and postope rative complications and surgical success rates at 6 months were evaluated. Results The mean age of the patients was lower in the TVT group than in the TOT group (46.2 + 6.1 and 53.3 + 8.1, respectively) (p 0.05). Operation time was longer in TVT group (19.7 + 4.3 min) than in TOT group (13.8 + 2.6 min) (p 0.05). Success rates were similar in both groups and no statistical difference was found (p> 0.05). Conclusion Both TVT and TOT can be used successfully and sa fely in the treatment of SUI, and there is no difference in efficacy.
