Mide kanserinde helicobacter pylori infeksiyonu ile sağkalım arasındaki ilişki
Amaç: Mide kanseri, kansere bağlı ölümlerin en sık nedenlerindendir. Mide kanserli hastalarda %60-70e varan oran-larda Helicobacter Pylori ( H. pylori) seropozitifiği tespit edilebilmektedir. H. pylori pozitifiği ile mide kanseri yer -leşim bölgesi ve prognoz arasında ilişki olduğunu öne süren çalışmalar yayınlanmıştır. Kliniğimizde takip edilenmide kanserli hastalarda, H. pylori pozifiği ve sağkalım arasındaki ilişki araştırılmıştır.Gereç ve Yöntem: Mide kanseri tanısı almış ve patoloji preperatlarına ulaşılabilen toplam 60 hasta çalışmaya alındı.Hastaların kliniko-patolojik özellikleri tarandı. Patoloji preperatları Giemza ile boyanarak H. pylori pozitifiği ve in-testinal metaplazi (İM) varlığı araştırıldı. Genel sağkalım analizinde Kaplan Meier yöntemi kullanıldı.Bulgular ve Sonuç: Patoloji preperatlarına ulaşılabilen 60 mide kanserli hastanın 34ünde (%56.7) H. pylori pozitif,26ında (% 43.3) negatif olarak saptandı. Toplam 13 (%21.7) hastada kardiya tümörü saptanırken, 47 (%78.3) hasta-da tümör yerleşimi kardiya dışında idi. H. pylori pozitif grubun %85inde tümör yerleşimi kardiya dışında iken, H.pylori negatif grupta bu oran %69.3tü. İntestinal metaplazi H. pylori pozitif olan hastaların % 66.6 sında, H. pylorinegatif olan hastaların ise %73.9unda pozitif olarak saptandı. İki grup arasında yaş (p:0.81), cinsiyet (p:0.76), yerle-şim yeri (p:0.20), evre (p:0.49) ve İM (p:054) açısından istatistiksel olarak anlamlı fark yoktu.
Relationship of helicobacter pylori ınfection and survival in gastric cancer patients
Introduction: Gastric cancer is one of the most common cause of cancer related death. In gastric cancer, H. pyloriseropositivity is around to be 60-70 %. Some studies reported that H. pylori positivity is related with overall survivaland cancer localisation. In this study, in gastric cancer patients, the relationship between H. pylori and overall sur -vival was examined.Material and Method: The pathologic specimens of 60 patients were evaluated for intestinal metaplasia and H.pylori positivity with Giemsa staining. The medical records of patiens were also examined for clinicopathologiccharecteristics. Survival rates were analysed with Kaplan Meier method.Results and Conclusion: Of 60 patients, H. pylori was positive in 34 (56.7%) and negative in 26 (43.3%). In 13 pa-tients (21.7%) tumor was located in cardia and in other 47 (78.3%) in non-cardia. Tumor was located in non-cardiac region in 85% of the H. pylori positive group and in 69.3% of theH. pylori negative group. Intestinal metaplasiawas found in 66.6% of the H. pylori positive group and 73.9% of the H. pylori negative group. There were nodifference between two groups in sense of age (p:0.81), gender (p:0.76), localisation (p:0.20), stage (p:0.49) andintestinal metaplasia (p:054)
