Salmonella türlerinin hızlı tanısında kromojenik besiyeri ile birlikte “Matrix-Assisted Laser Desorption Ionization - Time of Flight Mass spectrometry” yönteminin phoenix otomatize sistemi ile kıyaslanması

İnsan ve hayvanlarda gıda ile bulaşan hastalıklara yol açan etkenlerin en başta gelenlerinden biri olan Salmonella identifikasyonu genellikle 2-3 günden fazla sürmektedir. Kütle spektrometrisi esasına dayanan bir yöntem olan MALDI-TOF ile rutin mikrobiyoloji laboratuvarlarında kullanılan otomatize sistemlere göre daha kısa sürede, güvenilir ve maliyet etkin sonuçlar elde edilebilmektedir. Bu çalışmanın amacı, “Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry (MALDI-TOF) yönteminin Phoenix otomatize identifikasyon sistemi ile karşılaştırmasını yaparak bu patojen organizmaların yol açtığı gastrointestinal enfeksiyonlarda kısa sürede sonuç veren, pratik, maliyet etkin ve güvenilir bir yöntem olduğunun değerlendirilmesidir. Ağustos 2007 ve Ağustos 2011 tarihleri arasında toplanan 11186 hasta örneği çalışmaya dahil edilmiştir. Hasta örnekleri rutin olarak CHROMagar Salmonella plus ve Mac Conkey agar besiyerlerine ekilmişlerdir. 347 Salmonella türü hem Phoenix otomatize sisteminde hem de MALDI-TOF cihazında çalışılmıştır. 2 yöntemin sonuçları birbiri ile %100 uyumludur. Her iki cihazda da tür düzeyinde tanımlama yapılabilmekte, alttür tanımlamaları için konvansiyonel olarak Salmonella antiserumları ile serolojik tiplendirmeye gereksinim duyulmaktadır. CHROMagar Salmonella plus besiyerinin MALDI-TOF ile birlikte kullanımı identifikasyon için gereken zamanı azaltmış, Salmonella türlerinin kolaylıkla isimlendirilmelerini sağlamıştır. MALDI-TOF’un rutin klinik mikrobiyoloji laboratuvarında kullanımı Salmonella türlerinin tek bir besiyeri ile ve 48 saat içerisinde antibiyotik duyarlılıkları ile birlikte maliyet-etkin ve güvenilir bir şekilde saptanabilmesini sağlamaktadır.

Comparison of “Matrix-Assisted Laser Desorption Ionization- Time of Flight Mass Spectrometry” plus chromogenic media with phoenix automated system in the rapid diagnosis of Salmonella species

The identification of Salmonella which is a major food-borne pathogen generally takes more than 2-3 days. More reliable, rapid and cost effective results can be achieved with MALDI-TOF which is based on mass spectrometry principles in comparison with the automated systems in routine microbiology laboratories. The aim of this study is to evaluate the use of Matrix-Assisted Laser Desorption Ionization-Time of Flight Mass Spectrometry (MALDI-TOF) in gastrointestinal infections caused by those pathogen microorganisms by using practical cost-effective and reliable methods within a less time period. Between August 2007 and August 2011, 11186 stool specimens of patients were included to the study. Patient samples were routinely inoculated onto CHROMagar Salmonella plus and Mac Conkey agar media. 347 Salmonella species were run with both Phoenix automated system and MALDI-TOF. The results of both of the methods are 100% concordant with each other. Using Chromogar Salmonella plus media together with MALDI-TOF identification shortened the time, helped to detect Salmonella species easily. Species identifications are being performed in both of the systems, but identification of subspecies still require conventionally serological typing with Salmonella antiserums. MALDI-TOF will help the routine clinical microbiology laboratory to detect Salmonella rapidly with only one media and in 48 h together with their antibiotic susceptibility testings in a cost-effective and reliable manner.
