Preoperatif hastalarda HBsAg, anti-HCV ve anti-HIV seropozitifliği

Amaç: Kan yolu ile bulaşan insan immün yetmezlik virü-sü (HIV), hepatit B ve C virüs enfeksiyonları sağlık çalı- şanları için özellikle acil servis, laboratuvarlar ve cerrahi servislerde ciddi bir mesleki risk oluşturmaktadır. Sağlık çalışanları için aşılama ve sıkı biyogüvenlik önlemlerinin alınması kan kaynaklı enfeksiyonlardan korunmada temelilkedir. Ayrıca cerrahi girişim öncesi hastaların serolojiktarama testleri bulaş riskini azaltır. Bu çalışmada, preope- ratif hastalardaki HBsAg, anti-HCV, anti-HIV 1/2 seropre- valansını belirlemek amaçlanmıştır.Yöntemler: Ocak 2012-Aralık 2012 tarihleri arasında ameliyat hazırlığı döneminde preoperatif işlemler olarak HBsAg, anti-HCV, anti-HIV 1/2 araştırılan 4,367 hastanın kan örneği sonuçları geriye dönük olarak değerlendirildi. Bulgular: HBsAg pozitiflik oranı %7,7 (n=336), anti-HCV pozitiflik oranı %2,3 (n=101) olarak belirlenmiştir. Anti-HIV 1/2 pozitif saptanan 5 hastanın 2’si (%0.05) doğrulama testiyle pozitif saptanmış, doğrulama testi negatif 3 hasta yalancı pozitif olarak kabul edilmiştir. Sonuç: Kan ve vücut sıvılarıyla bulaşan hastalıklar hakkında tüm sağlık çalışanları eğitilmeli, HBV’ye karşı aşılanmalı, ameliyathanelerde enfeksiyon gelişimini engelleyecek evrensel önlemlerle ilgili standartlar ciddiyetle uygulanmalı ve temas edilen her hastanın enfekte olabi- leceği kabul edilmelidir.

Seropositivity of HBsAg, anti-HCV and anti-HIV in preoperative patients

Objective: The infections caused by human immunodefi- ciency virus (HIV), hepatitis B (HBV) and C (HCV) viruses pose a serious occupational risk for the healthcare work- ers especially those in emergency services, laboratories and surgery wards. Vaccination and establishment of the strict biosafety procedures are the main principles to pre- vent blood-borne infections in healthcare workers. Addi- tionally, serological screening of the preoperative patients could decrease the risk for exposure. In this study, we aimed to determine the seroprevalence of HBsAg, anti- HCV, anti-HIV 1/2 in preoperative patients. Methods: Hospital automation records were evaluated retrospectively for 4.367 patients who were scheduled for surgery and scanned for anti-HIV 1/2, HBsAg and anti- HCV as preoperative procedures in the preparation period of operation between January 2012 and December 2012. Results: HBsAg positivity rate was found in 7.7% (n=336), anti-HCV positivity rate was found in 2.3% (n=101). A two (0.05%) of five patients were positive for anti-HIV 1/2 was found positive verification test and the other three samples were accepted as false positive test results. Conclusion: All healthcare workers must be trained about occupational diseases and vaccinated against Hepatitis B. Universal precautions must be strictly followed particu- larly in the operating room. In addition, all patients should be considered as potential carriers regarded as a carrier of the potential for infection.
