Giriş: Elektif operasyonların iptali, ameliyathanenin verimli çalışmasına engel olmakta, hastane olanaklarının gereksiz kullanımına neden olarak ekonomik kayıplara yol açabilmektedir. Operasyon gününde iptalin en olumsuz örneği, hastanın operasyon salonuna alındıktan sonra operasyonun iptal edilmesidir. Bu durum ameliyathane çalışanlarının performansını olumsuz etkileyerek iş gücü kaybına neden olmakta, aynı zamanda hasta ve hasta yakınları üzerinde olumsuz psikolojik etkilere yol açmaktadır. Çalışmamızda retrospektif olarak, iptal edilen operasyonların kayıtları incelenmiş, iptal nedenlerinin ve önlenebilir olanların belirlenmesi amaçlanmış, ayrıca iptal sonrası cerrahi süreç araştırılmıştır. Gereç ve yöntem: Temmuz 2010-Aralık 2012 arasında elektif operasyonu planlanan ancak operasyon salonunda operasyonu iptal edilen 18 yaş ve üzeri tüm hastaların dosyaları retrospektif olarak incelendi. Operasyon iptal gerekçeleri ve iptal sonrası 1 aylık süreçte operasyonun gerçekleşip gerçekleşmediği kaydedildi. Bulgular: Belirlenen tarihler arasında operasyonu planlanan 22932 hastanın 365(%1.59)’inin operasyonu iptal edildi. Operasyonların 307’si(%84.1) anestezi uzmanı tarafından, 49’u ise (%13.4) cerrah tarafından iptal edilirken, 9 hasta ise (%2.5) kendi isteğiyle opere olmaktan vazgeçti. Operasyon iptallerinin 291’i(%79,7) regüle olmayan hipertansiyon, elektrokardiyografide anormal bulgu, solunumsal problemler, genel durum bozukluğu, laboratuar değerlerinde olumsuz değişiklikler gibi medikal koşullara bağlı olarak iptal edildi. İptal edilen 365 hastanın sonraki 1 aylık süreci incelendiğinde; 258(%70.7) hastanın operasyonlarının bu süre içerisinde anestezi altında gerçekleştiği görüldü. Sonuç:Çalışmamızda, operasyon salonunda iptallerin en sık anestezi uzmanı tarafından medikal koşullar nedeniyle gerçekleştirildiği sonucuna varılmıştır. Operasyon planlanan hastalarda, rutin preoperatif değerlendirme yanında, hastaların operasyondan bir gün önce servislerinde görülmesi, cerrahi ekiple iletişim içinde kalarak ortak kontrol listeleri üzerinden eksiklerin giderilmesi ile olası iptallerin azaltılabileceği kanısındayız.

Introduction: Cancellation of elective operation on the intended day of surgery not only decreases the over-all efficiency of the operation room but also negatively effects the economic burden due to the waste of resources. Most negative form of the cancellation of surgery is cancellation of the procedure after the patient has been admitted in the operation room. In this study, we retrospectively analysed the recordings of the cancelled surgeries in terms of the avoidable reasons and also we investigated the postcancellation course of the surgical procedures. Material and Method: We retrospectively analysed the recordings of the patients between July 2010 and December 2012 whom elective surgery was cancelled. Patients under 18 years old were not included in the study. The reason of cancellation and the re-do of the cancelled procedure throughout 1 month period was also investigated. Results: A total of 22932 elective surgical cases were scheduled for operation during the study period. In this period 365 (1.59 %) surgeries were cancelled. The most frequent causes for elective case cancellation were anesthetist related which accounts for 307 cases (84.1%), followed by surgeon-related 49 cases (13.4%) and unwilling patient-related 9 cases (2.5%). Medical conditions were the causes of 291 (79.7%) cancelled surgeries. In the postcancellation one month period of the 365 cancelled cases, 258 (70.7%) of them underwent surgery under anesthesia. Conclusion: Most frequent reason of the cancellation of the surgery were found to be anesthetist related due to the medical conditions. In addition to routine preoperative evaluation, assessment of the patient the day before the surgery at the ward, and clear comminication with surgeons may prevent and minimize the cancellation of surgery on the intended day.
