ULTRASONOGRAFİ EŞLİĞİNDE PERKÜTAN KARACİĞER BİYOPSİSİ; KLİNİK TECRÜBEMİZ VE VERİLERİMİZ
Giriş: Görüntüleme yöntemleriyle tanı konulamayan karaciğer hastalıklarında, malignite evrelemesinde ve tedavi planlamasında karaciğer biyopsisi gerekmektedir. Bu çalışmamızda kliniğimizde yaptığımız ultrasonografi (USG) eşliğinde perkutan karaciğer biyopsilerinin sonuçlarını ve komplikasyon oranlarını değerlendirmeyi amaçladık. Gereç ve Yöntem: Kliniğimizde Mart 2016-Şubat 2019 dönemleri arasında USG eşliğinde perkütan karaciğer biyopsisi yapılan 112 hasta retrospektif olarak incelendi. Bulgular: Kliniğimizde 76’sı (%67.9) karaciğer parankim biyopsisi, 36’sı (%32.1) karaciğer kitle biyopsisi olmak üzere 112 hastaya karaciğer biyopsisi yapıldı. Hastaların 50’si (%44.6) kadın, 62’si (%55.4) erkekti. Yaş ortalaması 56,3’tü. Biyopsilerin 108’i (%96.4) patolojik tanı için yeterliydi. 2 hastada (%1.7) subkapsüler hematomperihepatik sıvı ve 4 hastada(%3.4) medikal tedaviyle gerileyen ağrı olmak üzere hastaların 6’sında (%5.1) minör komplikasyonlar saptandı. Sonuç: Sonuç olarak klinik tecrübemize göre USG eşliğinde yapılan perkutan karaciğer biyopsileri hızlı, etkili, minimal invaziv ve maliyet etkindir. Komplikasyon oranları oldukça düşüktür. Tüm hastalarda karaciğer biyopsilerinin ultrason eşliğinde yapılması gerektiğini düşünüyoruz.
ULTRASOUND GUIDED PERCUTANEOUS LIVER BIOPSY; OUR CLINICAL EXPERIENCE AND DATA
Introduction: Liver biopsy is required in cases that can not be diagnosed by imaging methods, malignancy staging and treatment planning. We aimed to evaluate percutaneous liver biopsies, results and complication rates in our clinic. Material and Method: A total of 112 patients undergoing ultrasonound (US) guided percutaneous liver biopsy, performed between March 2016 and February 2019, were reviewed retrospectively. Results: In our clinic, liver biopsy was performed in 112 patients, 76 of them (67.9%) were liver parenchymal biopsy and 36 of them (32.1%) were liver mass biopsy. Fifty (44.6%) of the patients were female and 62 (55.4%) were male. The mean age was 56.3 years. 108 (96.4%) of the biopsies were sufficient for pathological diagnosis. Minor complications were detected in 6 (5.1%) of the patients, including subcapsular hematoma-perihepatic fluid in 2 patients (1.7%) and pain relieved with medication in 4 patients (3.4%). Conclusion: In conclusion, US guided percutaneous liver biopsies are fast, effective, minimally invasive and cost effective procedures in our clinical experience. Complication rates are very low. We think that liver biopsies should be performed under ultrasound guidance in all patients.
