Mediopatellar plikada manyetik rezonans görüntülemenin artroskopik bulgularla karşılaştırılması

Amaç: Artroskopide saptanan mediopatellar plikaların (MP) manyetik rezonans görüntüleme (MRG) raporlarında varlığı araştırıldı. Dizdeki efüzyon derecesi ile MP’nin artroskopi ve MRG sonuçları arasındaki farklılık değerlendirildi. Aynı zamanda MP için artroskopide kullanılan sınıflama yöntemlerinden biri olan Dandy sınıflamasının MRG’de uygulanabilirliğini incelendi. Yöntem: Şubat 2005-Mayıs 2006 arasında kliniğimizde artroskopi uygulanan dizlerden MP tanısı alanların MRG’leri retrospektif olarak incelendi. MRG’deki görünümleri artroskopi esnasında MP tiplendirmesi için kullanılan Dandy sınıflamasına göre yapıldı. Dizlerdeki efüzyonun miktarı üç seviyede derecelendirildi. Artroskopik sonuçlarla MRG sonuçları karşılaştırıldı. Bulgular: Toplam hasta sayısı 53’dü (32 erkek, 21 kadın). Hastaların opere edilen dizlerinin 30’u sağ, 23’ü sol dizdi. Ortalama yaş 37.65 yıldı. MRG rapor sonuçları geriye dönük incelendiğinde 16’sında (% 30.2) plikanın varlığına ait veri bulundu. MP bulunan 53 hastanın 28’inde (% 52.8) artroskopik tiplendirme sonuçları ile MRG tiplendirmesinin sonuçları arasında uyum bulundu. Efüzyon evre I’de artroskopi ile MRG sonuçları arasında anlamlı fark vardı (P0.05). Sonuç: Diz ekleminde efüzyonun artması ile MRG’de MP’nın görülme oranı artmaktadır. Bu çalışma ile Dandy sınıflamasının MRG’de MP’nın tanımlanmasında objektif verilere dayandığı için kullanılabileceğini düşünmekteyiz.

Comparison between MR imaging results and arthroscopy findings of plica synovialis mediopatellaris

Objective: The purpose of this study was to determine whether mediopatellar plicae (MP) document in the magnetic resonance imaging (MRI) report which diagnoses with arthroscopic examination. We evaluated the differences between the arthroscopic findings and MRI reports with the level of knee effusion. And we also observed availability of Dandy arthroscopic classificationfor MP with MRI evaluation. Methods: We investigated the MRI&#8217;s of the patients retrospectively who underwent arthroscopic examination determine the MP from February 2005 to May 2006 in our clinic. We used Dandy arthroscopic classification for MP of MRI. We determined the effusion level of the knee with three grades. Arthroscopic and MRI results were compared. Results: Total patients number was 53. Knees were examined in 32 men and 21 women. There were 30 right 23 left knees. The average age at operation was 37.65 yr. When the reports of the MRIs examined retrospectively we found data about the existence of MP in 16 (% 30.2) of them. In 28 patients (% 52.8) accordance was found between the arthroscopic and MRI classification results. There was a statistically significant difference between arthroscopic and MRI results for grade I effusion (p<0.05). There was no statistically significant difference between arthroscopic and MRI results for grade II and III effusion (p>0.05). Conclusion: By the increase of the effusion in knee joint, the incidence of MP increases in MRI examination. In this study we think that Dandy classification could be used for determining MP with MRI because of its objective data.
