Periorbital bölgedeki bazal hücreli kanserlerin tedavisi: 54 olgunun incelenmesi

Amaç: Bu çalışmada periorbital bölgenin bazal hücreli kanserlerin tedavi yöntemlerinin tartışılması amaçlanmıştır. Yöntem: Periorbital bölgesinde bazal hücreli kanser olan 54 hasta (61 bazal hücreli kanser) tedavi edildi. Hastalara primer tamir, deri grefti, kondromukozal greft ve lokal flep yöntemleri ile toplam 67 onarım yapıldı. Bulgular: Primer tamirlerde dokular anatomik karşılıklarına dikkat edilerek dikildiğinde başarılı sonuçlar alındı. Doku defektlerinin onarımında lokal flepler benzer özellik taşıdığından ilk olarak tercih edildiler. Greftle onarımlarda ilk tercih olarak kulak arkasından tam kalınlıkta deri grefti kullanıldı. Dört vakada inceltilmeden uygulanan glabellar flepler periorbital bölge için kalın bulundu. Sonuç: Periorbital bölge bazal hücreli kanserlerin en sık görüldüğü yerlerdendir. Bu bölge vücudun küçük bir kısmını oluşturmasına karşın fonksiyonel ve estetik açıdan birçok önemli yapı içermektedir. İstenilen fonksiyonel ve estetik sonuçlara ancak bölgenin anatomisinin iyi bilinmesi, ameliyat öncesi iyi değerlendirme yapılması ve uygun onarım yöntemi seçilmesi ile ulaşılır.

Management of periorbital basal cell carcinomas: Analysis of 54 cases

Objective: The aim of this study was to discuss the repair methods of the periorbital basal cell carcinoma. Methods: 54 patients (61 basal cell carcinomas) with periorbital basal cell carcinomas were treated. 67 repairs were carried out in soft tissues by primary repair, skin graft, chondromucosal grafts and local flaps. Results: Successful results were obtained in primary repairs when reciprocal places sutured. Local flaps were preferred in tissue defects because of similar structure. Full thickness skin graft harvested from the back of the ear was chosen firstly in skin graft repairing and color harmony was achieved. Glabellar flaps that performed without thinning in four cases were detected thick in periorbital region. Conclusion: Periorbital basal cell carcinomas are common. Periorbital area comprises many important functional and aesthetical units despite being a little portion in whole body. Desired functional and aesthetic results may be attained to know the anatomy of this area, good evaluation of the lesion preoperatively and choose the suitable repair method.
