A pediatric case with idiopathic pulmonary fibrosis
İdiyopatik akciğer fibrozisi çocuklarda nadir görülen ciddi bir hastalıktır. Kronik bir progresyon gösterir ve genellikle ölümle sonuçlanır. Akciğerlerde intertisyel fibrozis ve alveolo kapiller membranda defekt bulunur. Biz sekiz aydır olan nefes darlığı ve tekrarlayan öksürüğü nedeniyle pediatrik allerji bölümüne başvuran bir hastayı sunduk. Akciğerlerin muayenesinde yaygın ince railer duyuldu. Akciğer grafisinde her iki akciğerin orta ve alt zonlarında infilttasyon görüldü. Yüksek rezolusyonlu komputerize tomografide apikal zonlar dışında her iki akciğerde buzlu cam görünümü saptandı. Transbronşiyal ve açık akciğer biyopsisinde diffüz intertisyel fibrozis görüldü. Solunum fonksiyon testleri restriktif patern ile uyumlu idi. Akciğer fibrozisinin etyolojisi, yapılan bütün testler ile açıklanamadığı için vaka İdiyopatik akciğer fibrozisi olarak kabul edildi ve hastaya kortikosteroid tedavisi başlandı. Tedaviye belirli zamanlarda dozu azaltılarak 1.5 yıl devam edildi. Solunum fonksiyon testinde zorlu vital kapasite %46'dan %72'ye, 1. saniyede zorlu ekspitatuvar volüm %49'dan %75'e yükseldi. Kortikosteroid tedavisine hastanın klinik ve laboratuar bulgularına göre azaltılarak 1.5 yıl devam edildi Hasta kortikosteroid tedavisinden fayda gördü. Klinik bulguları düzeldi akciğerlerdeki railerde azalma saptandı. Yüksek rezolusyonlu komputerize tomografideki buzlu cam görünümü tedavinin ilk yılının sonunda geriledi.
Idiopathic pulmonary fibrosis is a severe lung disease that appears very rarely in children. It shows a chronic progress and results generally in death. Interstitial fibrosis and a defect in alveolar-capillary gas exchange are present in the lungs. We report a patient who presented to our Pediatrie Allergy Depart-ment due to recurrent cough and dyspnea of 8 months duration. Widespread cracking rales were present in respiratory system examination. Diffuse infiltration in the middle and lower zones of both lungs on pulmonary radiography was observed. High resolution computerized tomography revealed ground glass appearence in both lung areas except the apical zones. The results of transbronchial biopsy and open lung biopsy demonstrated a diffuse interstitial fibrosis. The results of pulmonary function tests were consistent with the restrictive pattern. Case was accepted as idiopathic pulmonary fibrosis since all of the tests directed to the possible underlying etiology of pulmonary fibrosis were unrevealing. Systemic steroid treatment was started following diagnosis of Idiopathic pulmonary fibrosis. The treatment has been continued for 1.5 years by tapering the dose in accordance with the clinical and laboratory improvements of the patient Corticosteroid treatment decreased the crackles in lungs and clinical improvement was observed. The Forced vital capacity ratio increased from %46 to %72 and Forced expiratory volume in 1 sec ratio from %49 to %75 in the pulmonary function tests. Ground glass apperance on High resolution computerized tomography was determined to regress at the end of the first year of treatment.
