Amaç: Musculus abductor pollicis longus'un (MAPL) yüzeyel parçasının tendonu I.dorsal bilek kompartmanından geçerek basis ossis metacarpalis I 'de sonlanır. Derin parçasının tendonu ise os trapezium'a tutunur. MAPL'un articulatio carpometacarpalis I' in stabilizasyonundaki önemli rolü nedeniyle morfolojik özellikleri önem kazanmaktadır. Aksesuar tendonların sayısı, kalınlığı ve uzunluğu de Quervain'ın stenozan tendovaginitinin gelişmesinde fonksiyonel öneme sahiptir. Ayrıca aksesuar tendonlar plastik rekonstrüksiyonlar için transplantasyon kaynağı olarak değerlendirilebilir. Çalışmamızın amacı MAPL'un farklı insersiyo tiplerini değerlendirmek ve klinik önemini vurgulamaktır. Gereç ve Yöntem: Formalin ile fikse edilmiş erişkin insan kadavralarından elde edilen 87 üst ekstremitede MAPL'un tendonlarının tutunma yerleri ve sayıları araştırıldı. Bulgular: MAPL'nin basis ossis metacarpalis I, os trapezium ve m.abductor pollicis brevis'e tutunan üç tendonlu örneği 21 olguda (%24.14) gözlendi. MAPL'un basis ossis metacarpalis I ve m.abductor pollicis brevis'e tutunan iki tendonlu örneği 20 olguda (%22.99), basis ossis metacarpalis I ve os trapezium'a tutunan iki tendonlu örneği 20 olguda (%22.99) belirlendi. Sonuç: MAPL'un İnsersiyo varyasyonlarının bilinmesi klinik uygulamalara yardımcı olacaktır.

Objective: The superficial part of the abductor pollicis longus muscle (APLM) passes along the first dorsal compartment of the wrist and inserts into the base of the metacarpal bone of the thumb. The tendon of the deep part inserts into the trapezium. Since the APLM has an important role on the stabilisation of the first carpometacarpal joint, its morphological characteristics are becoming more important. The number, thickness and length of accessory tendons have a functional significance in the development of de Quervain's stenosing tendovaginitis. On the other hand these tendons may be regarded as a source of transplant material for plastic reconstructions. The aim of this study is to appraise the different insertion types of the APLM and emphasize its clinical importance. Material and Method: The number and the insertion sites of the tendons of the APLM was investigated in 87 upper extremities taken from formalin fixed adult human cadavers. Results: The APLM insertion type with three tendons, one inserting to the base of the metacarpal bone of the thumb, the other to the trapezium and the last to the abductor pollicis brevis muscle, was observed in 21 cases (24.14%). The sample of the APLM which had two tendons, one inserting to the base of the metacarpal bone of the thumb and the other to the abductor pollicis brevis muscle, was found in 20 cases (22.99%). A different type of the APLM insertion with two tendons, one inserting to the base of the metacarpal bone of the thumb and the other to the trapezium was observed in 20 cases (22.99%). Conclusion: Knowledge of the variations of the insertion types of the APLM will help the clinical applications.
