Hidradenitis süppürativa

Hidradenitis süppürativa özellikle genç kadınlarda görülen ve belli ölçülerde yaygın olan bir hastalıktır. Apokrin ter bezi olan bölgede yerleşen bu hastalık, ağrılı bir nodülden apse, sinus oluşumu ve hipertrofik skarlara kadar değişen bir semptom kompleksinden oluşur. Klinik bulgular arasında, apokrin ter bezlerinin yerleşmiş olduğu bölgelerde ağrılı nodüller, sinus yolları ve hipertrofik skarlar vardır. Bazı risk faktörleri olmasına karşın nedeni tam olarak bilinmemektedir. Orta şiddetteki olgular bile yaşam kalitesini etkiler. Tedavi medikal ve cerrahi olup kliniğe göre bireyselleştirilmesi gerekir. Geniş spektrumlu antibiyotik ve eksizyon girişimleri tutulumunun genişliğine göre ayarlanır. Diğer yandan anti-TNFa ve retinoidler gibi bazı ilaçlar etkin olmakla beraber fayda-risk oranı açısından halen soru işaretleri vardır. Bu makalede bu sorunun sunumu yapılarak patogenezi ve tedavisi derlenmiştir.

Hidradenitis suppurativa

Hidradenitis suppurativa is somewhat common entity that affect especially young women. It consists in a symptoms complex with presentations ranging from painful nodule(s), to abscesses, sinus tracts and hypertrophic scars that are located in the apocrine gland-bearing areas. There are some risk factors, but the cause is unknown. Quality of life is severely affected even in the mild forms. Treatment is both medical and surgical and needs to be individualized to the clinical presentation. Wide-spectrum antibiotics and excisions tailored to the extent of involvement. On the other hand, some drugs such as anti-TNFa drugs and retinoids are effective, but their risk-benefit ratio is still in question. This article reviews the presentation, pathogenesis and treatment of this problem.
