Blastoschizomyces capitatus’a bağlı fungal pnömoni
Blastoschizomyces capitatus, nadir rastlanan fakat mortalitesi yüksek bir invazif mantar infeksiyonu etkenidir. Günümüze kadar bildirilmiş 100 civarında olgu mevcuttur. Bu mantara bağlı infeksiyonlar için risk faktörleri uzamış nötropeni, agresif kemoterapi, kortikosteroit kullanımı, geniş spektrumlu antibiyotik kullanımı, deri ve mukoza bütünlüğünün bozulması sonucu lokal savunma mekanizmalarının değişikliğe uğramasıdır. Bu yazıda lenfoma tanılı bir hastada B.capitatus’a bağlı pnömoni olgusu sunulmuştur. Serum ve BAL’da galaktomannan testi pozitif olduğu için bu olgu önemli bulunmuştur.
Pneumonia due to a fungal pathogen: Blastoschizomyces capitatus
Blastoschizomyces capitatus is a rare but frequently fatal cause of an invasive fungal infection. About 100 cases have been reported until today. Risk factors for systemic B.capitatus infection have been reported as prolonged neutropenia, intensive chemotherapy, corticosteroid treatment, the use of broad-spectrum antibacterial agents and local disruption of skin and mucosal defenses. We reported a fungal pneumonia due to B.capitatus in a patient who had lenfoma. Because galactomannan assay was positive in BAL and serum samples, this case was considered to be important.
