Nozokomiyal santral sinir sistemi infeksiyonlarında mortaliteyi etkileyen faktörler
Nozokomiyal santral sinir sistemi (SSS) infeksiyonları, hastane infeksiyonları arasında insidansının düşük olmasına rağmen, mortalitesinin ve morbiditesinin yüksek olması nedeniyle önemini korumaktadır. Bu çalışmada, 2003-2009 yılları arasında gelişen nozokomiyal SSS infeksiyonlarında mortalite üzerine etkili olan faktörler incelenmiştir. Veriler infeksiyon kontrol komitesi tarafından prospektif olarak toplanmış, değerlendirme ise retrospektif olarak yapılmıştır. Nozokomiyal SSS infeksiyonları toplam 48 hastada saptanmış ve en sık beyin cerrahisi bölümünde gözlenmiş ve toplam nozokomiyal infeksiyonların % 0.85’inden sorumlu olarak bulunmuştur. Nozokomiyal SSS infeksiyonlarında mortalite % 33 olarak saptanmıştır. Genç yaşta olma, invazif girişimler, eksternal ventriküler drenaj (EVD) kateteri varlığı ve Gram negatif patojenlerle gelişen infeksiyon mortalite artışı ile ilişkili bulunmuştur. Gram pozitif patojenlerle gelişen infeksiyonlarda ise hayatta kalma oranı anlamlı derecede yüksek bulunmuştur. Ancak multivariat analiz yapıldığında EVD kateteri varlığı ve genç yaşta olma mortalite için bağımsız risk faktörleri olarak saptanmıştır. Çalışmamızda en sık izole edilen etkenler koagülaz negatif stafilokoklar ve Acinetobacter baumannii olarak saptanmıştır. Karbapenem dirençli olan Gram negatif patojenlerle gelişen infeksiyonlarda mortalite anlamlı derecede yüksek saptanmıştır (p=0.021). Nozokomiyal SSS infeksiyonları hastanemizde sık görülmemekle birlikte yüksek mortaliteye sahiptir. Genç yaşta olma, invazif girişimler ve EVD kateteri varlığı mortalite riskini artırmaktadır. Karbapenem dirençli Gram negatif patojenlerle infeksiyon gelişmesi mortaliteyi artırmaktadır ve bu nedenle empirik tedavi seçiminin sürveyans verileri doğrultusunda yapılması uygun olacaktır.
Factors associated with mortality in nosocomial central nervous system infections
Although central nervous system (CNS) infections have relatively low incidence among the nosocomial infections, their mortality and morbidity are high. We aimed to analyze the mortality risk in nosocomial CNS infections between 2003-2009. The data were prospectively collected by the infection control committee by active surveillance, and the analysis was performed retrospectively. A total of the 48 patients with nosocomial CNS infection was detected during the study period. They have been observed most commonly in the Neurosurgery Unit. Nosocomial CNS infections were responsible from 0.85 % of the total nosocomial infections. The mortality was found to be 33 % in nosocomial CNS infection in this study. By comparison between fatal and survived cases, younger age, invasive procedures, external ventricular drainage (EVD), and the presence of gram negative pathogens were associated with mortality. In the infections developed by gram positive pathogens, survival rate was found to be significantly higher. In a multivariate analysis, younger age and EVD were independent risk factors associated with mortality. Coagulase negative staphylococci and Acinetobacter baumannii were the most frequent isolates in our study. The mortality were significantly higher in infections caused by carbapenem resistant gram negative isolates (p=0.021). Nosocomial CNS infections were uncommon in our hospital, however, their mortality rates were high. Younger age, the presence of invasive procedures and EVD catheter increase the risk for mortality. The development of CNS infections with carbapenem resistant gram negative pathogens increased the mortality. Therefore, the choice of empirical treatment should be directed by local surveillance data.
