Listeria monocytogenes’e bağlı bir menenjit olgusu
Listeria monocytogenes toplumumuzda sık görülmeyen bir infeksiyon hastalığı etkenidir. Bazı özel hasta gruplarında hayatı tehdit eden infeksiyonlara neden olabilmektedir. Ateş yüksekliği, uykuya meyil, bulantı-kusma yakınmaları olan 78 yaşında erkek olgu hastaneye yatırıldı. Olgunun beyin omurilik sıvısı (BOS) incelemesinde; görünüm opelasan, basınç artmış, 350 lökosit/mm3 (% 60 lenfosit, % 40 PNL), şeker 16 mg/dL (eş zamanlı kan şekeri 136 mg/dL), protein 258 mg/dL olarak saptandı. BOS örneğinin Gram boyası ile incelemesinde Gram pozitif çomaklar görüldü. BOS kültüründe L.monocytogenes izole edildi. Olgu, ampisilin+gentamisin kombinasyonu ile tedavi edilerek şifa ile taburcu edildi.
Menengitis due to listeria monocytogenes: Case report
Listeria monocytogenes is a pathogen that is not a common cause of infections in our population. However it can lead to life treatening infections in some special patient groups. 78 years old male patient is hospitalised with symptoms such as fever, lethargy, nausea and vomiting. The findings of cerebrospinal fluid (CSF) analysis were opalescence appearance, increased pressure, 350 leucocyte/mm3 (60 % lymphocyte, 40 % PNL), glucose 16 mg/dL (simultaneously blood glucose 136 mg/dL), and protein 258 mg/dL. Gram positive bacteria were seen in preparation and L.monocytogenes grew in culture of CSF. The patient healed by administration of ampicillin+gentamicin combination.
