A grubu beta-hemolitik streptokoklarda eritromisin direnci
Bakteriyel akut tonsillofarenjitin en sık sebebi A grubu beta-hemolitik streptokoklar (GABHS)’dır. Akut tonsillofarenjitin tedavisinde klasik olarak penisilin ve türevleri kullanılmaktadır. Ancak penisilin allerjisi olanlarda veya penisiline karşı tolerans varlığında alternatif tedavi seçeneği olarak makrolidler önerilmektedir. Çalışmamızda Ekim 2007 - Mart 2008 tarihleri arasında akut üst solunum yolu infeksiyonu ön tanısı konmuş hastaların boğaz kültürlerinden izole edilen GABHS’larda eritromisin direnci araştırılmıştır. GABHS olarak tanımlanan bakteri örneklerinin eritromisine direnci CLSI M100-S16’da belirtildiği şekilde disk difüzyon yöntemiyle belirlenmiştir. Çalışılan 1570 boğaz sürüntü örneğinin 142’sinden (% 9) GABHS izole edilmiş ve bunların 12’sinde (% 8) eritromisin direnci saptanmıştır. Eritromisine karşı direnç oranları bölgemizde daha önce yapılan bir çalışmaya göre daha yüksek bulunmuştur. Sonuç olarak; penisilinin kullanılamadığı GABHS infeksiyonlarında antibiyogram sonuçlarına göre tedavinin planlanmasının uygun olacağı düşünülmüştür.
Erythromycin resistance in group A beta-hemolytic streptococci
Group A beta-haemolytic streptococci (GABHS) are the most common causes of acute bacterial tonsillopharyngitis. For streptococcal tonsillopharyngitis, the classical therapeutic choice is penicillin and its derivates. However, in cases of penicillin allergy or intolerance, macrolides are recommended as alternative drugs. The aim of this study was to determine the erythromycin resistance in GABHS isolated from throat cultures of pa-tients with upper respiratory tract infections between October 2007-March 2008. Erythromycin susceptibility of GABHS strains was determined by agar disk diffusion technique according to the Clinical Laboratory Standards Institute (CLSI M100-S16) guidelines. A total of 142 (9 %) GABHS strains were isolated from 1570 throat samples and the resistance to erythromycin was found in 12 (8 %) strains. The data of resistance rates to erythromycin is relatively higher than that obtained by another study previously performed in our region. According to this result, when penicillin could not be used, the antimicrobial treatment of upper respiratory tract infections due to GABHS should be planned according to antimicrobial susceptibility testing.
