Monitorize anestezi bakımında propofol ve deksmedetomidinin hemodinamik ve bispektral indeks değerleri açısından karşılaştırılması
Amaç: Çalışmamızda el cerrahisi ameliyatlarında, propofol ve deksmedetomidinin, monitorize anestezi bakımı sırasındaki sedasyon skorları ile hemodinamik ve bispektral indeks (BİS) değerlerini karşılaştırmayı amaçladık. Yöntem: Elektif el cerrahisi planlanan 40 hasta rast gele iki eşit gruba ayrıldı. Sedasyon indüksiyonu Grup D'de 1 fig kg1 deksmedetomidin İV (10 dakikada), Grup P'de ise 1 mg kg$kg^{-1}$ propofol İV (10 dakikada) yapılarak tamamlandı. İndüksiyonu takiben Grup D'ye 0.6-0.7 $mu$ $kg^{-1}$ $sa^{-1}$ dozda deksmedetomidin, Grup P'ye ise 3 mg $kg^{-1}$ $sa^{-1}$ dozda propofol infüzyonu başlandı. Sedasyonun başlangıcından sonra ortalama kan basıncı, kalp atım hızı, dakika solunum sayısı, periferik oksijen satürasyonu (SpO2), sedasyon ve ağrı skorları ile Bispektral indeks (BİS) değerleri kaydedildi. A comparison of the BIS values of propofol and dexmedetomidine in monitored anesthesia care
Objective: In our study we aimed to compare the sedative and hemodynamic effects of propofol and dexmedetomidine by bispectral index during hand surgery. Method: Patients (n=40) undergoing elective hand surgery were randomized into two groups. In group D, sedation was induced with 1 fig $kg^{-1}$ of IV dexmedetomidine (in 10 minutes) followed by an infusion rate of 0.6-0.7 $mu$ $kg^{-1}$ $hour^{-1}$. In group P, induction was done with 1 mg $kg^{-1}$ of IV propofol (in 10 minutes), followed by an infusion rate of 3 mg $kg^{-1}$ $hour^{-1}$. İntra- and postoperative sedation scores, mean blood pressure, heart rate, respiratory rate, peripheral oxygen saturation, pain scores and Bispectral index (BIS) were recorded. Results: The mean blood pressure and heart rate were significantly lower in the dexmedetomidine group (p<0.05). The intraoperative sedation scores and BIS values were significantly lower in the propofol group (p<0.05). Postoperatively they were found to be significantly lower in the dexmedetomidine group (p<0.05). Conclusion: In monitored anesthesia care, dexmedetomidine provides similar sedative effects compared to propofol. However, onset and recovery of sedation were both longer; and blood pressure and heart rates were lower in the dexmedetomidine group. The sedation scores were parallel to the BIS values.
