Yeme bozuklukları ve psikoz

Yeme bozukluklarında psikoz komorbiditesi anksiyete ve duygudurum bozukluklarından çok daha az incelenmiştir ve bu konudaki bulgular büyük oranda olgu serileri ile sınırlıdır. Bununla birlikte yeme bozuklukları olan hastalarda yapılan çalışmalar, genel popülasyonda şizofreni görülme oranı %0.8-1.2 iken, yeme bozukluğu olan hastalarda şizofreni gelişme riskinin %3-10 arasında olduğu ve şizofreni ve diğer psikotik bozuklukların bulimiya nervozadan kısıtlayıcı tip anoreksiyada üç kat, tıkınırcasına yeme-çıkarma tipi anoreksiyada iki kat daha fazla görüldüğünü bildirmiştir. Hastaların bir kısmında açık bir şekilde sanrı ve varsanılarla olan psikoz varlığının yanı sıra, daha büyük bir hasta grubunda gözlenen hastalığın açık ve yoğun bir şekilde yadsınması, ilişkilerden geri çekilme, affektif küntlük, düşünce rijiditesi ve obsesyonalizm, paranoid düşünceler, bozulmuş beden algısının subsanrısaldan sanrısala kadar değişen niteliklerde olabilmesi diğer yeme bozukluklarına göre anoreksiya nervozayı psikoza daha fazla yaklaştırmakta ve tedavisindeki asıl zorlukları oluşturmaktadır

Eating disorders and psychosis

Comorbidity of anxiety and mood disorders were much more studied than comorbidity of psychosis in the eating disorders and findings on this issue largely limited to case series. However, studies demonstrated that the incidence of schizophrenia is 0.8-1.2% in the general population, while in the patients with eating disorders is 310%. Schizophrenia and other psychotic disorders reported that 3-fold increase in restrictive type anorexia and 2fold increase in binging-purging type anorexia compared to bulimia nervosa. The minority of patients exhibit the presence of a clearly psychosis with delusions and hallucinations in eating disorders. However anorexia nervosa more close to psychosis than other eating disorders with a much larger parts of the patients exhibit clear and intense denial of the disease, withdrawal of relations, restricted affect, rigidity of thought and obsessionality, paranoid thoughts, distorted perception of the body ranging from subdelusion to delusion. This feature of disorder constitutes the main challenges in the treatment
