Spontan perinefrik ürinoma ile birlikte prostat adenokarsinoması: Olgu sunumu ve literatürün gözden geçirilmesi

Ürinoma perirenal veya paraüreteral bölgede gözlenebilen nadir bir durumdur. Obstrüktif üropatiler, toplayıcı sistem veya üreterde yaralanmaya yol açan kunt veya delici yaralanmalardan sonra meydana gelebilir. Bu yazıda, prostat kanseri olan 72 yaşındaki bir erkek olguda saptadığımız perinefrik ürinomayı sunuyoruz. Olgu üroloji polikliniğine idrar yapamama ve sağ yan ağrısı ile başvurdu. Parmakla rektal muayenede prostat sert ve düzensiz olarak palpe edildi. Bilgisayarlı tomografi ve manyetik rezonans görüntüleme tetkiklerinde sağ subkostal bölgeden pelvise kadar uzanan büyük bir perinefrik kistik kitle görüldü. Kist sıvısının biyokimyasal incelemesi ile kitlenin ürinoma olduğuna karar verildi. Sağ nefro-üreterektomi ve ürinoma eksizyonu operasyonu yapıldı. Bu konudaki literatür gözden geçirilerek patofizyolojisi ve tedavisi tartışıldı.

Prostate adenocarcinoma with spontaneous prinephric ürinoma: Case report and review of the literatüre

Urinoma is a rare condition that generally located at the perirenal or paraüreteral regions. It can occur from several reasons e.g. obstructive uropathies, blunt or penetrating trauma leading to injuries of the collecting system or ureter. Here, we report a case of perinephric urinoma in a 72 year-old man who had a prostate cancer. The patient was admitted to our urology clinic with unable to urinate and right lumbar pain. He had an irregular and firm prostate on digital rectal examination. Computed tomography scan and magnetic resonance imaging methods demonstrated a right large perinephic cystic mass located from the right subcostal area to the true pelvis. The biochemical investigation of cyst fluid was confirmed that this was an urinoma. Right nephro-ureterectomy and urinoma excision operations were performed. The literature on this subject is reviewed and pathophysiology and management of this condition are discussed.
