Tetik başparmak: Doğumsal, gelişimsel ve geç bir sağlık sorunu olarak

1989-2009 tarihleri arasında takip ettiğimiz 8 olguyu sunduk. Hastaları geriye doğru değerlendirdik. Değerlen-dirmeye arşiv fotoğraf kayıtlarımız esas teşkil etti. Hastalarımızdan 2’si erkek 6’sı kadın idi. İki hastada patoloji 40 yaşında ortaya çıkmıştı. Bu iki hasta da kadındı ve birisi lohusalık dönemi içindeydi. Diğer hastalardan birinin problemi doğumdan beri var olmakla beraber 9 yaşına kadar ameliyat edilmemişti. Dokuz yaşındaki bu kız çocuğunda da diğer çocuk hastalarda olduğu gibi ameliyatı genel anestezi altında yaptık. Erişkin hastalarımızda lokal anestezi altında çalıştık. Diğer çocuk hastalardan sadece bir tanesinde başpar-maktaki tetikleşme çocuk bir yaşında iken annesi tara-fından fark edilmişti ve ısrarla sormamıza rağmen anne sorunun doğumdan 1 yaşına kadar kesinlikle olmadığını söyledi. Diğer dört hastanın sorunları doğumla beraber vardı. Bir hastamızda sorun iki yanlı olarak mevcuttu. Bu yazı ile tetik başparmak patolojisinin daha çok doğumsal bir patoloji olarak dikkatimizi çekmekle beraber, süt çocuğu döneminde ve sonraki yıllarda da gelişimsel bir patoloji olarak da karşımıza çıkabileceğini vurgulamak istedik. Bebeklikten beri var olmasına rağmen 9 yaşına kadar tedavi edilmeyen olguda gözlemlediğimiz başpar-mak hipoplazisi dikkate alındığında tedavi edilmeyen olgularda gelişmeye ait sorunlar ortaya çıkabileceğine dikkat çekmek istedik.

Trigger thumb: As a congenital, developmental or late health problem

We presented 8 cases associated with trigger thumb. We evaluated the patients retrospectively. The evaluation was based on archive pictures. Two patients were women and 6 patients were men. The pathology existed at 40 year old in two of the patients. Two of them were women and one of them with in puerperal term. The left patients except one had the pathology from the first day of their lifetime. Triggering on the thumb appeared at one age on the exceptional case and her mother denied any congenital pathology in spite of our repeated questions. One of the case among the congenitally triggered thumb patients had not been operated on up to nine year old for several reasons. We performed the surgery under general anesthesia in pediatric patients. We did the operation under local anesthesia in adult patients. One of the pediatric patient had the problem bilaterally. We would like to emphasize that trigger thumb may exist as a late lifetime problem as much as a congenital or developmental pathology. We would like also to emphasize that in untreated and long standing cases a minimal thumb hipolasia may be expected by considering one of our cases.
