Artroskopik subakromiyal dekompresyonun fonksiyonel sonuçları ve enjeksiyon testiyle ilişkisi

Amaç: Bu çalışmanın amacı Neer evre 2 omuz sıkışma sendromu nedeniyle subakromiyal dekompresyon uygu lanan hastalarda, subakromiyal enjeksiyon testine yanıt ile üst ekstremite fonksiyonlarındaki değişim arasındaki ilişkinin değerlendirilmesidir. Hastalar ve yöntem: Çalışmaya en az 6 ay süreli konservatif tedaviden fayda görmemiş ve evre 2 omuz sıkışma sendromu olan 15 hasta dahil edildi. Tüm hastalara ameliyat öncesi dönemde subakromiyal enjek siyon testi uygulandı. Hastaların ağrı şiddeti 10 cm’lik görsel analog skalası (GAS) ile ölçüldü. Artroskopik subakromiyal dekompresyon öncesi ve sonrası, üst ekstremite fonksiyonu Disabilty of Arm, Shoulder and Hand anketinin Türkçe versiyonu (DASH-T) kullanılarak incelendi. Cerrahi girişimden bir gün sonra tüm hastalar standart bir rehabilitasyon programına alındı. Bulgular: Hastaların yaş ortalaması 42,9±8,9 yıldı ve sekizi erkekti. The functional outcome of arthroscopic subacromial decompression and its relationship with injection test

Objective: The objective of this study was to determine the relationship between the preoperative subacromial enjection test and change in upper extremity function after arthroscopic subacromial decompression in patients with Neer stage 2 impingement syndrome. Patients and methods: Fifteen patients with stage 2 shoulder impingement syndrome who have no benefit from at least 6 months of conservative treatment were included in the study. Subacromial injection was preoperatively administered to all patients. The severity of the patients’ pain was measured on a 10 cm visual analog scale (VAS). Before and after arthroscopic subacromial decompression, function of upper extremity was assessed using the Turkish version of Disabilty of Arm, Shoulder and Hand Questionnaire (DASH-T). All patients were involved in a standart rehabilitation program one day after surgical intervention. Results: The mean age of the patients was 42.9±8.9 years and eight of them were male. Before and after subacromial injection, VAS scores were determined as 8.5±1.1 and 2.3±0.9, respectively. A significant reduction in the patients’ pain was observed (p<0.01). The DASH- T score decreased from 93.2±8.9 preopera-tively to 52.6±6.27 at 6 weeks and 34.6±2.61 at 6 months postoperatively. Statistically significant differences were found between the pre and postoperative DASH-T scores (p<0,05). However, there was no correlation between the reduction in pain severity by subacromial injection and the improvement in upper extremity function at 6 months after surgery. Conclusion: The functional outcome of arthroscopic subacromial decompression was found to be succesfull in patients with stage 2 shoulder impingement syndrome who had no reesponse to conservative treatment. Nevertheless, the results of the present study indicates that the subacromial injection test is not an useful tool for predicting outcome after arthroscopic decompression.
