Daha önce komplet situs inversus tanısı almamış torasik kifoskolyozlu 51 yaşındaki bayan hasta, koroner arter hastalığı (KAH) şüphesi ile Teknesyum-99m metoksiizobutilizonitril (Tc99m-MIBI) rest-stres myokard perfüzyon single photon emission computed tomography (SPECT) görüntüleme için kliniğimize gönderildi. Bruce protokolüne göre koşu bandı ile egzersiz stres testi uygulandı. Sol hemitoraks’tan standart elektrokardiyografi elde edildi. Görüntüler 180 derece sirküler yörüngede standart sağ anterior oblik (RAO) projeksiyondan başlayan, sol posterior oblik (LPO) projeksiyonda sonlanan şekilde elde edildi. Rekonstrükste imajlarda anteroseptal, septal ve inferoseptal duvarda stres imajlarında daha belirgin olmak üzere perfüzyon defekti mevcuttu. Kardiyak oryantasyonun daha iyi sağlanması için rest-stres myokard perfüzyon SPECT görüntüleme bir hafta sonra tekrarlandı. SPECT görüntüleri 180 derece sirküler yörüngede sol anterior oblik (LAO) projeksiyondan başlanan, sağ posterior oblik (RPO) projeksiyonda sonlancak şekilde elde edildi. İkinci çalışmadan sonra rekonstrükte imajlarda perfüzyon defekti izlenmedi. Koroner anjiografi inceleme bulguları normal sınırlarda olarak değerlendirildi. Vakada ilk çalışmada izlenen perfüzyon defektinin; anatomik kardiyak malpozisyon, vertebral deformasyon ve aküzisyon artefaktı, yanlış yerde kullanılmış standart görüntüleme tekniği gibi sebeplere bağlı olabileceği düşünüldü. Ayrıca 180 derece sirküler yörüngede LAO projeksiyondan başlayıp, RPO projeksiyonda bitirilen görüntülemenin dekstrokardi vakalarında daha uygun görüntüleme tekniği olduğu sonucuna varıldı.

A 51-year-old female patient without a previous diagnosis of situs inversus with mirror image dextrocardia and kyphoscoliosis underwent Technetium-99m methoxyisobutylisonitrile (Tc-99m-MIBI) rest stress myocardial perfusion single photon emission computed tomography (SPECT). Treadmill exercise test was performed according to Bruce protocol. Standard electrocardiography images were recorded from left hemithorax as usual. Images were obtained on the circular orbit of 180 degrees starting from right anterior oblique (RAO) projection and ending at left posterior oblique (LPO) projection. Tc-99m-MIBI rest stress myocardial perfusion SPECT images revealed abnormal perfusion with reversible perfusion defects in the anteroseptal, septal and inferoseptal walls. To ensure a better cardiac orientation the rest-stress myocardial imaging was repeated from a different angulation one week later. New 180 degree circular orbit SPECT images were started from left anterior oblique (LAO) projection and terminated at right posterior oblique projection (RPO). There were no perfusion defects in those images. Coronary angiography performed one week after SPECT study was normal. Although there was no evidence of coronary artery disease in angiograpy in this case, the false-positive reversible perfusion defect might be because of acquisition artifacts, cardiac malposition, vertebral deformity, misused standart imaging technique etc. Because of mirror-image dextrocardia in our patient, Tc-99m-MIBI myocardial SPECT applied using 180 degree arc, starting from LAO projection and ending at RPO projection and ending at RPO provided more suitable images.
