Bacak kas kitlesinin belirlenmesinde Tc-99m-sestamibi sintigrafisinin kullanım

Amaç: Kas doku miktarı değişimlerinin tüm vücutta ve bölgesel olarak ölçümü beslenme, spor ve egzersiz fizyolojisinde önem taşımaktadır. Çalışmamızda DEXA ekstremitede iskelet kas kitlesinin ölçümünde standart yöntem olarak kullanıldı ve sistemik hastalığı olmayan farklı yaş ve cinsiyetlerdeki bireylerde periferik iskelet kas kitlesini belirlemede MIBI sintigrafisinin değerini araştırmayı amaçladık. Gereç ve Yöntemler: Koroner arter hastalığı için 10 yıllık riski ≤%10 olan 119 kadın ve 83 erkek (Toplam 202) hasta çalışmaya alındı. DEXA ile bacak ve tüm vücut yağ ve kas doku kitlesi (kg) ile % yağ oranları; MIBI ile MIBI uptake ve Normalize MIBI indeksleri hesaplandı. Bulgular: Hem DEXA hem de MIBI ile elde edilen değerlere göre erkeklerde kas kitlesi kadınlara göre anlamlı olarak fazla idi (sırasıyla; p == 0.0001,

Use of Tc-99m-sestamibi scintigraphy for the determination of leg muscle mass

Objective: Whole body and regional measurement of the amount of the muscle mass changes (is important) in nutrition, fitness and exercise physiology. The measurement of limb skeletal muscle mass with DEXA is the standard method in our study, and the purpose of this study was to determine the value of MIBI scintigraphy in measuring the peripheral skeletal muscle mass in a population of different ages and gender without systemic disease. Material and Methods: Total of 202 patients (119 women and 83 men) whit a 10-year risk of coronary artery disease &#8804; 10 are enrolled in this study. The leg and total body fat and muscle tissue mass (kg) and % fat with DEXA and MIBI uptake and Normalized MIBI indices with MIBI were calculated. Results: According to the values obtained by both DEXA and MIBI, mens muscle mass was significantly higher than women&#8217;s (p = 0.0001, p< 0.01, respectively). The muscle mass values obtained by DEXA and MIBI were not different in men of different age groups, however DEXA values was highest in women at the 3rd decade, and were significantly different between women&#8217;s age subgroups (p<0.05). The lower limb muscle mass obtained by DEXA was found to have a significant negative relationship with age, HDL cholesterol level, % lower extremity fat and % of total body fat. The data obtained with Gated SPECT were significantly different between male and female patients (p = 0.0001). Between resting and pharmacological test status, EF values were significantly different only in female patients. Conclusion: In our study, the regional muscle mass measurement values obtained by DEXA and MIBI gave consistent results. According to both methods, the muscle mass in men was higher than women, and fat mass was less. At the 3rd decade, muscle mass was at maximum, decreasing at menopause in women. MIBI can be used for noninvasive in vivo measurement of regional muscle mass under special circumstances.
