Birinci basamak sağlık kuruluşlarında solunum sistemi semptomlu hastalarda atopi ne zaman araştırılmalı?

AMAÇ: Atopik hastalıklar IgE aracılı bir aşırı duyarlılık reaksiyonudur. Kronik solunum sistemi semptomlu hastalarda atopik inceleme önemlidir. YÖNTEM: GATA Allerjik hastalıklar polikliniğine 2006 yılında alt solunum yolları (ASY) ve üst solunum yolları (ÜSY) şikayetleri ile müracat eden atopi araştırması yapılmamış 92 hastada atopik incelemesi yapıldı. BULGULAR: Çalışmamızda deri prick testlerinde bir veya daha fazla allerjene pozitiflik 62 hastada (%67) saptandı. ÜSY semptomu olarak 4/yıl ve az atak geçiren 31 hastanın 18’inde (%58), 4’ten fazla geçiren 33 hastanın 24’ünde (%73) atopi saptandı. ÜSY semptom süresi 4 haftadan kısa 16 hastanın 5’inde (%31), 4 haftadan uzun süren 48 hastanın 30’unda (%63) atopi saptandı. ASY semptomu olarak 4/yıl ve az atak geçiren 24 hastanın 6’sında (%25), 4’ten fazla geçiren 12 hastanın 4’ünde (%33) atopi saptandı. ASY semptom süresi 4 hafta/atak ve az süren 14 hastanın 3’ünde (%21), 4 hafta/atak’tan fazla süren 22 hastanın 16’sında (%73) atopi saptandı. SONUÇ: Yıllık üst ve/veya alt solunum yolları enfeksiyonu 4’ten fazla, semptom süreleri 4 haftadan uzun süren hastalarda atopi araştırmasının yapılmasını önermekteyiz.

When atopic research must be done in respiratory system diseases in primary health care?

AIM/BACKGROUND: Atopic diseases are conditions related to IgE-mediated hypersensitivity. Atopic research is important in patients who have chronic respiratory disease symptoms. METHODS: Ninety-two patients with symptoms of respiratory system diseases, which were evaluated at GATA Allergy Clinic, were studied. All patients were researched as atopy respects. RESULTS: Cutaneous reactivity to one or more allergens was detected in 18 subjects of 31 patients (%58), who had upper respiratory symptoms in 4 and less attack/ year and 24 subjects of 33 patients (%73), who had more than 4 attack in a year. Atopy was detected in 5 subjects of 16 patients (%31), who had upper respiratory symptom duration 4 and less week / year, 30 subjects of 48 patients (%63), who had more than 4/ year. Atopy was detected in 3 subjects of 14 patients (%21), who had a lower respiratory symptom duration 4 and less week/year and 16 subjects of 22 patients (%73), who had more than 4 week/year. CONCLUSION: We recommend evaluation of atopy for patients who have upper and/or lower respiratory system infections more than 4 attacks in a year, and symptoms duration time more than 4 weeks.
