Gebelerin Sürekli Ebe Liderliğinde Evde Doğum Hakkında Bilgi ve Görüşleri

Amaç: Bu çalışmanın evde vajinal doğum oranlarını arttırdığı çalışmalarla kanıtlanan ve Dünya Sağlık Örgütü (DSÖ)'nün önerdiği ELSBM ile gebelerin Ebe Liderliğinde Evde Doğum (ELED) hakkında bilgi ve görüşlerini belirlemek amacıyla yapılmıştır. Yöntem: Çalışma kesitsel tanımlayıcı tipte olup bir üniversite hastanesinin kadın doğum polikliniklerine 23 Mart- 20 Temmuz 2019 tarihleri arasında gebelik kontrolü için başvuran ve çalışmaya katılmayı kabul eden 260 gebe ile tamamlanmıştır. Elde edilen verilerin istatistiksel değerlendirmesi bilgisayar ortamında SPSS 22.0 paket programı ile yapılmıştır. Bulgular: Gebelerin eğitim durumları ile, ELSBM ile vajinal doğum (p=0,000), epizyotomisiz doğum (p=0,000) ve evde doğum (p=0,000) yapabileceğini düşünmeleri arasında istatistiksel olarak anlamlı bir fark bulunmaktadır. Eğitim seviyesi arttıkça vajinal, epizyotomisiz ve evde doğum yapabileceğini düşünme oranı da artmaktadır. Sonuç: ELSBM ile sürekli ebe liderliğinde bakım ile evde doğum yapabileceğini düşünen kadınların oranının istenilen düzeyde olmadığı görülmektedir ancak gebelerin neredeyse yarısının modelin maternal riskleri azaltacağını, yarısından daha fazlasının yenidoğan risklerini azaltacağını ve bebeğini daha etkili emzirmede etkili olacağını düşünmelerinin saptanması oldukça dikkat çekicidir.

The Knowledge and Opinions among Pregnant Women about Home Birth by Midwife-led Continuity Model of Care

Objective: This study was conducted to determine the knowledge and opinions of pregnant women about Midwife-led Home Birth (MLHB) by Midwifeled Continuity Model of Care (MLCMC), which has been recommended by World Health Organization (WHO) and proven through the studies to increase the rates of vaginal delivery at home. Method: This was a cross-sectional descriptive study and carried out among 260 pregnant women admitted to the obstetrics and gynecology outpatient clinics of a university hospital between 23rd March and 20th June 2019 and agreed to participate in the study. The statistical analysis of the data was performed using SPSS 22.0 computer program. Findings: There was a statistically significant difference between the educational status of the pregnant women and their beliefs that they could perform vaginal delivery (p = 0,000), birth without episiotomy (p = 0,000) and home birth (p = 0,000) by MLCMC. As the educational level increased, the rates of believing that they could give birth vaginally, without episiotomy and at home also increased. Conclusion: It was seen that the ratio of women believing that they could give birth at home by Midwife-led Continuity Model of Care (MLCMC) is not at the desired level, however, it is quite remarkable to observe that almost half of the pregnant women believe that MLCMC will reduce maternal risks, more than half of them think that it will reduce new-born risks and will be influential in breastfeeding their babies more effectively.
