Açık kalp cerrahisi sonrası magnezyum infüzyonunun etkileri

Ameliyat sonrası dönemde hipomagnezemi oldukça sıktır ve klinik olarak anlamlı bir morbidite sebebidir. Magnezyum verilmesinin kalp ritmi, debisi ve ameliyat sonrası kanama üzerine etkisini saptamak amacıyla prospektif, kontrollü klinik bir çalışma gerçekleştirildi. Valvular kalp cerrahisi sonrası yoğun bakımda, 12 hastaya 3 gr magnezyum sülfat, 500 mi %5 dekstroz içerisinde, 12 saatlik infüzyonla; 12 hastaya ise aynı sürede 500 mi %5 dekstroz verildi. The effects of magnesium infusion after open heart surgery

Postoperative hypomagnesemia is common in patients who have undergone cardiac operations and is associated with clinically significant morbidity. We performed a prospective, controlled clinical study to investigate the effect of magnesium supplementation on cardiac rhythm, output, and postoperative bleeding. In intensive care unit after valvular heart surgery, 12 patients were randomized to receive an intravenous infusion of magnesium sulphate, 3 gr in 500 ml 5% dextrose, and 12 patients received only 500 ml 5% dextrose within 12 hours after the termination of cardiopulmonary bypass. Ventricular extrasystoles were observed in 2 magnesium treated patients and 7 control patients (p<0.05). They also had better cardiac indices (magnesium treated group: 2.85±0.2 vs control group: 2.53±0.1 L/min per m2, p<0.05 at 6th hours). There was no difference between groups according to their total chest tube drainage (magnesium treated group: 427±123 ml vs control group: 444±109, p>0.05). As a result, postoperative magnesium administration decreased the frequency of postoperative dysrhytlhmias, increased the cardiac indices and did not influence the hemorrhagic status.
