Venöz Tromboemboli Hastalarında Gen Mutasyonları ve Risk Faktörlerinin Değerlendirilmesi
Amaç: Bu çalışmanın amacı idiyopatik pulmoner emboli saptanan ve sekonder nedenlere bağlı olarak ortaya çıkannonidiyopatik pulmoner emboli olgularında gen mutasyonlarının varlığını ve dağılımını karşılaştırmaktır.Yöntem ve Gereçler: Pulmoner emboli tanısı konulan 57 hasta retrospektif olarak incelendi. Hastalar sekondernedenlere bağlı olarak nonidiopatik pulmoner emboli (nPE) ve idiopatik pulmoner emboli (iPE) olarak iki gruba ay- rıldı. Her iki gruptaki hastaların özgeçmişleri, trombozis risk analizleri ve risk faktörleri retrospektif olarak incelen- di. Gen mutasyonu taşıma durumları gruplar arasında karşılaştırıldı.Bulgular: Her iki grup arasında mutasyonların varlığı açısından anlamlı farklılık saptanmadı (p=0,16). Tüm hasta- larda %63 oranında en az 2 trombofili faktörü olduğu görüldü. Tüm hasta grubunda hastaların trombozis risk analiziincelendiğinde en sık PAI-1 4G/5G (%57,6) ve MTHFR C677T (%42,1) mutasyonu izlendi. 6 (%10,5) hastada genmutasyonu izlenmedi.Sonuç: Bu çalışmada; nPE hastalarında iPE ile benzer oranda gen mutasyonu saptanmamakla beraber tüm hastalar- da %63 oranında uzun süre antikoagülan tedavi gerektirecek en az 2 trombofili faktörü olduğu görüldü. Bu açıdanbakıldığında rutin genetik inceleme yapılması pahalı bir tetkik olması nedeni ile tedavi maliyetini artıracaktır; ancakseçilmiş vakalarda kar zarar hesabı yapılarak genetik inceleme yapılması faydalı olabilir. Assessment of Gene Mutations And Risc Factors In Patients With Venous Thromboemboli
Aim: The aim of this study is to compare presence and distribution of gene mutations in patients with idiopathic pul- monary emboli and nonidiopathic pulmonary emboli due to secondary causes. Material and Methods: Fifty seven patients diagnosed as pulmonary emboli, were analysed retrospectively. The patients were divided into 2 groups, one idiopathic pulmonary emboli (iPE) and the other was nonidiopathic pulmo- nary emboli (nPE). nPE cases were arised from secondary causes. Medical history of all patients, thrombosis riskanalysis and risk factors were analyzed retrospectively. Gene mutation status were compared between two groups.Results: There was no statistically significant difference between two groups in terms of presence of gene mutations(p=0,16). At least 2 thrombophilia factors were found in 63% of all patients. PAI-1 4G/5G (%57,6) and MTHFRC677T (%42,1) mutations were the most common mutations when thrombosis risk was analysed. No gene mutationwas detected in 6 (10,5%) patients.Discussion: In this study, similar gene mutation rates were determined in nPE and iPE patients. On the other hand atleast 2 thrombophilia factors were detected in 63% of all patients in both groups and this situation would require longtime anticoagulant therapy.When considered from this point of view; due to high prices, routine genetic analysiswill increase the treatment costs. But in selected cases, genetic analysis may be useful when cost effective analysiswere done.
