Normal Koroner Arterli Hastalarda Trombosit Dağılım Genişliği Koroner Yavaş Akım İle İlişkiliyken Ortalama Trombosit Hacmi ilişkili Değildir

Amaç: Koroner Yavaş Akım (KYA), anjiografide ciddi epikardiyal darlık olmaksızın opak maddenin distale yavaş geçişi olarak tanımlanır. Altta yatan mekanizmalar net olmamakla beraber, enflamasyon, artmış trombosit aktivitesi, endotel disfonksiyon ve mikrovasküler disfonksiyonun patogenezde rol aldığı düşünülmektedir. Bu çalışmada ortalama trombosit hacminin (OTH) ve trombosit dağılım genişliğinin (TDG) KYA' a etkisini araştırmayı amaçladık.Yöntem ve Gereçler: Çalışmaya anjiyografide normal koroner akım saptanan elli hasta (20 erkek, ortalama yaş: 53 ± 11 yıl) ve yine anjiyografide üç koroner arterde de KYA saptanan 50 hasta (21 erkek, ortalama yaş: 57 ± 10 yıl) alındı. Tüm hastaların koroner akım hızları, sineanjiyografi ile 25 kare/saniye hızında belirlenen TIMI (Thrombolysis In Myocardial Infarction) kare sayısı (TKS) yöntemi kullanılarak hesaplandı. Hemoglobin, beyaz küre, trombosit sayısı ve kırmızı hücre dağılım genişliği (KHDG) her iki grupta da benzerdi. Platelet Distribution Width But Not Mean Platelet Volume is Associated with Coronary Slow Flow Phenomena in Patients with Normal Epicardial Coronary Arteries

Aim:Coronary slow flow (CSF) is delayed distal contrast opacification of coronary arteries in the absence of significant epicardial stenosis and, inflammation, increased platelet activation, endothelial and microvascular dysfunctions are thought to be involved in the pathogenesis. We aimed to evaluate the effect of both the mean platelet volume (MPV) and platelet distribution width (PDW) on CSF.Material and Methods: Fifty patients with angiographically proven normal coronary flow (20 men, mean age: 53 ± 11 years) and 50 patients with angiographically proven slow coronary flow in all three coronary arteries (21 men, mean age: 57 ± 10 years) were included in the study. The coronary flow rates of all patients were measured by the Thrombolysis In Myocardial Infarction frame count method with cineangiography at 25 frames per second. Mean platelet volume, platelet distribution width and other hematologic indices were compared between these groups. No significant differences were noted in hemoglobin, white blood cell, platelet counts and red cell distribution width between groups. Also, MPV values were similar (p=0.1).
