Gebelerde grup B streptokok kolonizasyonu ve antimikrobiyal duyarlılıkları

Amaç: Bu çalışma; fetal ölüm, yenidoğanda sepsis, menenjit, pnömoni, osteomiyelit gibi tablolara; annede sepsis, postpartum endometrit, amniyonit, nekrotizan fasiit gibi ciddi enfeksiyonlara neden olabilen B grubu streptokok (GBS) taşıyıcılık oranının belirlenmesi amacıyla planlanmıştır. Gereç ve Yöntem: Bu çalışmada, hastanemiz Kadın Hastalıkları ve Doğum Polikliniğine başvuran 12 hafta ve üzerinde gebeliği olan 120 gebe incelendi. Örnekler iki eküvyonlu çubuktan birincisi ile vagina arka duvarından, dış üretral delikten ve anorektal bölgelerden alınarak Todd–Hewitt buyyonlu sıvı besiyerine ve diğeri ile ise vaginadan örnek alınarak transport besiyerine (Stuart) kondu. Transport besiyerine alınan vagina örneklerinden kanlı, EMB ve SDA besiyerlerine ekim yapılırken, Todd-Hewitt besiyerindeki örnekler 37oC’de aerop ortamda 18-24 saat süreyle inkübe edildikten sonra %5 koyun kanlı agara subkültürleri yapıldı. İnkübasyon sonunda üremeler, rutin mikrobiyolojik yöntemlerle değerlendirildi. Bu amaçla CAMP testi (standart suş S.aureus ATCC 25923), sodyum hippurat hidrolizi deneyi, %6.5 NaCl’de üreme, basitrasin ve trimetoprim- sülfometoksazol (TMP-SXT) direnci araştırılarak kökenler GBS olarak tanımlandı. Bulgular: Çalışma kapsamına alınan 120 gebenin sadece 9’unda (%7.5) GBS izole edildi. Bu izole edilen GBS’lerin 7’si vaginadan, 1 tanesi dış üretral delikten, 1 tanesi de rektumdan alınan örneklerde rastlandı. Bütün suşlar tetrasikline dirençli iken, seftriakson, vankomisin, siprofloksasin, sefotaksim, oflaksasin, eritromisin, klindamisin, kloramfenikol, ampisilin ve penisiline karşı direnç saptanmamıştır. Sonuç: GBS, gebelik sırasında saptanması ve uygulanacak tedavi ile özellikle bebek ölümlerin ve ağır komplikasyonların önlenmesi açısından önem taşımaktadır.

Colonization of group B streptoccocal bacteria in pregnants and their antibiotic susceptibility

Objective: This study was planned to investigate the portage ratio of Group B streptococus (GBS) in pregnant women which is related with fetal death, sepsis, menengitis, pneumonia, osteomyelitis in neonatal period and serious maternal infections such as sepsis, postpartum endometritis, amniotis, necrotisan faciitis. Material and Method: 120 women, at 12 weeks or later gestation who were admitted to outpatient clinics were enrolled into the study. Group B streptococus cultures were taken with double ecuvoined sticks; initially from the posterior wall of vagina, external urethral meatus and anorectal region and were put it in Todd- Hewitt bouillon agar and the second sampling were made from the vagina and put it in transport agar (Stuart). Samples from transport agar were cultured in blood, EMB and SDA agars and samples in Todd-Hewitt agar were incubated at aerob Colonization of Group B Streptoccocal Bacteria In Pregnants and Their Antibiotic Susceptibility conditions with 37 oC’de for 18-24 hours and then subcultured with 5% sheep bloody agar. Proliferations after incubation period were assessed with microbiological methods. For this purpose strains were defined as GBS after investigating for CAMP test (standart strain S.aureus ATCC 25923), sodium hippurat hidrolisis method, proliferating in 6.5% NaCl, bacitracin and trimetoprim sulfometoksazol (TMP-SXT) resistance. Results: GBSwas isolated in only 9 of 120 pregnant women (7.5%) and 7 of these samples were taken from vagina,one from external urethral meatus and one from rectum. All strains were found as resistant to tetracycline and susceptible to ceftriaxone, vancomycin, ciprofloxacine, cephotaxim, ofloxacine, erythromycin, clindamicine, chloramphenicol, ampicilline and penicillin. Conclusion: Detection and treatment of GBS during pregnancy could be associated with prevention of neonatal deaths and serious complications.
