Clinical Outcomes of Deliveries Complicated with Maternal Cardiac Disease: Single Center Experience
Amaç: Bu çalışmanın amacı, gebeliği sırasında bir kalp hastalığıyla komplike olmuş ve üçüncü basamak sevk mer- kezi olan merkezimizde gerçekleşen doğumların anne ve bebek sağlığı açısından klinik sonuçlarını incelemektir.Yöntem ve Gereçler 2009 ve 2012 yılları arasında kliniğimize ait tüm doğum verileri geriye dönük olarak tarandı.Daha önceden kalp hastalığı olan veya gebeliği sırasında bir kalp hastalığı teşhisi konan ve kliniğimizde doğum ya- pan gebeler çalışmaya dahil edildi.Bulgular: 1500 doğumdan 9 unun bir maternal kalp hastalığıyla ilişkili olduğu saptandı. Kliniğimizde kalp hastalığıyla komplike doğum sıklığı %0.6 olarak saptandı. Sezeryan seksiyo ile doğum yapan bir hastada yapay kalp kapakhastalığı trombozuna bağlı gelişen akciğer ödemi sonucu hem anne ve hem de bebek ölümü geçkeleştiği; 2 hastadaorta derecede nitral yetersizliği ve orta derecede pulmoner hipertansiyon; 2 hastada atriyal septal defekt; bir hasta- da orta derecede sol ventrikül sistolik disfonksiyon; bir hastada triküspit kapak hastalığı ve bir hastada koroner arterhastalığı saptandı. Kalp Hastalığı olan 9 gebeden 2 si normal vajinal doğum ve 7 si Sezeryan seksiyo ile doğum yap- tı. Kliniğimizde kalp hastalığı ile komplike olmuş gebeliklerin doğumlarıı. Maternal Kalp Hastalığı ile Komplike Doğumların Klinik Sonuçları: Tek Merkez Tecrübesi
Aim: The study aimed to investigate the maternal and fetal outcomes of pregnancies which are complicated withmaternal cardiac disease or diagnosed a heart disease during pregnancy and delivered in our tertiary referral center. Material and Methods: Between 2009 and 2012, data of deliveries of our clinic were screened retrospectively. Pa- tients who had heart disease before and also diagnosed a heart disease during pregnancy and delivered in our clinicwere included the study.Results: Nine of 1500 deliveries were associated with a maternal cardiac disease. Prevalence of deliveries whichcomplicated with a heart disease was 0.6% in our clinic. In one patient who was delivered by cesarean section, pul- monary edema occurred due to prosthetic heart valve thrombosis and resulted both maternal and fetal death; moder- ate degree mitral regurgitation and pulmonary hypertension in 2 patients; atrial septal defect in 2 patients; moderatedegree left ventricular systolic dysfunction and congestive heart failure in one patient; tricuspid valve disease in onepatient and coronary artery disease in one patient. Two of nine subjects with heart disease were delivered by normalvaginal route and 7 patients were delivered by cesarean section. Maternal and fetal mortality rates of pregnanciescomplicated withheart diseasewere 11% in our clinic. Conclusion: Prevalence of heart disease during delivery is 0.6% and maternal-fetal mortality rates are 11% in ourclinic.
