Gastrik Polipler: Doğu Akdenizde 3. Basamak Gastroenteroloji Merkezi Sonuçları

GİRİŞ ve AMAÇ: Üst gastrointestinal sistem endoskopilerinde mide polibi saptadığımız hastaların demografik verileri ve poliplerin özelliklerini değerlendirmek. YÖNTEM ve GEREÇLER: Geriye dönük olarak, 2014-2018 yılları arasında hastanemiz Gastroenteroloji bilim dalında, üst gis endoskopilerinde mide polibi saptadığımız hastalar veri kayıt sisteminden tespit edildi. Hastaların demografik özellikleri, poliplerin lokalizasyonları, büyüklükleri, polibin alınma şekli ve gelişmişse komplikasyonları, histopatolojik özellikleri ve malignite potansiyelleri değerlendirildi. BULGULAR: Bu süre içerisinde toplam 13905 hastaya endoskopi yapılmıştı. Çalışmaya alınan 70 hastanın midesinde toplam 100 polip saptanmıştı. Hastaların %61’i kadın olup yaş ortalaması 63,1±24,6 yıl idi. %49 erkek olup yaş ortalamaları 61,7±17,3 yıl idi. Polipler biyopsi forsepsiyle ya da koter kullanılarak polipektomiyle çıkarılmışlardı. İşleme bağlı komplikasyon gelişmemişti. Poliplerin ortalama büyüklüğü 1,87±0,87 cm idi. Polipler %41,4 hastada antrumda, % 40 hastada korpusta, % 10 hastada fundusta, %5,7 hastada kardiada, %2,9 hastada pilorda saptanmıştı. Poliplerin %60’ı hiperplastik polip olup, %10’u adenomatöz polip, %8,6’sı fundik gland polibi, %7,1’i nöroendokrin tümör, %5,7’i hamartamatöz polip, %4,2’i inflamatuar fibroid polip idi. Hiperplastik poliplerin %9,5’unda düşük dereceli displazi mevcuttu. Hastalarımızdan sadece tübülovillöz adenom, yüksek dereceli displazi şeklinde olan hastanın takibinde mide adenokanseri gelişmişti. TARTIŞMA ve SONUÇ: Hastalarımızda en sık görülen gastrik polip tipi hiperplastik poliplerdi. Mide polipleri kadınlarda ve orta ileri yaşlarda daha yaygındı. Poliplerinin çoğu 1 cm'den küçük olup çoğunlukla antrum ve korpusta lokalize idi.

Gastric Polyps: 3rd Stage Gastroenterology Center Results in the Eastern Mediterranean

INTRODUCTION: To evaluate the demographic data and the characteristics of polyps in patients with gastric polyps in upper gastrointestinal endoscopy. METHODS: Retrospectively,in 2014-2018,patients with gastroenterology in our department of gastroenterology and upper gis endoscopies were determined from the data recording system.The demographic features of the patients, the localization of polyps,their size,the way the polyp was taken and its complications,histopathological features and malignancy potentials were evaluated. RESULTS: During this period, a total of 13905 patients had endoscopy.A total of 100 polyps were detected in the stomach of 70 patients included in the study.61% of the patients were women and the average age was 63.1±24.6 years.49% were male and mean age was 61.7±17.3 years.All polyps were removed either by biopsy forceps or polypectomy using cautery.Procedural complications did not develop.The average size of the polyps was 1.87±0.87 cm.Polyps were detected in the antrum in 41.4% patients, in the corpus in 40%,in the fundus in 10%,in the cardia in 5.7%,in pyloria in 2.9%.60% of the polyps are hyperplastic polyps,10% are adenomatous polyps,8.6% fundic gland polyp,7.1% neuroendocrine tumor,5.7% hamartamatous polyp,4.2% inflammatory fibroid polyp was.Low-grade dysplasia was present in 9.5% of hyperplastic polyps.Gastric adenocarcinoma developed in the follow-up of the patient, who was only in the form of tubulovillous adenoma-high-grade dysplasia. DISCUSSION AND CONCLUSION: In our patients, the most common histology type of gastric polyp was hyperplastic polyps. Stomach polyps were more common in women and middle-advanced ages, most of the gastric epithelial polyps were less than 1 cm long, and were most often localized in the antrum and corpus.
