Pulmoner aspergillomalı olgularımız

Amaç: Pulmoner aspergilloma akciğer kaviteleri içinde yerleşebilen Aspergillus türlerinin neden olduğu mikotik bir hastalıktır. Kliniğimizde takip ettiğimiz 5 pulmoner aspergillomalı olgunun özelliklerini sunduk. Olgu sunumu: 3’ü erkek, 2’si kadın olan olguların yaşları 35-67 arasında idi. Olgularımızın üçü iyileşmiş akciğer tüberkülozu, biri ise epidermoid karsinom tanısı almıştı. Hemoptizi ve öksürük en sık belirlenen semptomlardı. Sonuç: Geçirilmiş akciğer tüberkülozlu, özellikle de steril kavitesi bulunan olgularda hemoptizi, aspergillomadan şüphe ettirmelidir. Aspergillomalı hastalar klinik takibe alınmalı, masif hemoptizi olmadıkça cerrahi tedavi düşünülmemelidir.

Our cases with pulmonary aspergilloma

Objective: Pulmonary aspergilloma is a mycotic disease which can be sited in lung cavities and caused by Aspergillus species. We have reported five cases of pulmonary aspergilloma followed in our clinic. Case report: Of the five cases, three were male and two were female. Age was ranging between 35 and 67. Three cases were lung tuberculosis which had been cured; one case was diagnosed as epidermoid lung cancer. Hemopthysis and cough were the most frequent symptoms. Conclusion: If the patient having hemopthysis has sterile cavity due to previous pulmonary tuberculosis, aspergilloma should be suspected. Aspergilloma cases should be followed up and unless massive hemopthysis occurs surgery should not be performed.
