Massif mide kanamasına yol açan bir Dieulafoy lezyonu (olgu sunumu)

Amaç: Dieulafoy lezyonu nadir, fakat potansiyel olarak hayatı tehdit eden gastrointestinal kanama nedenidir. Bu vasküler anomali sıklıkla proksimal midede görülmekle beraber özofagus, ince barsak ve kolonda da bulunabilmektedir. Olgu Sunumu: Yaklaşık bir haftadan beri tekrarlayan hematemez atakları olan hastaya yapılan özofagogastroskopide mide proksimalinde posterior duvarda akut kanayan vasküler yapı görüldü ve skleroterapi uygulandı. Hastanın kanamasının tekrarlaması nedeniyle gastrotomi+wedge rezeksiyon+primer sütür uygulandı. Rezeksiyon materyalinin histopatolojik incelemesi sonucunda “Dieulafoy lezyonu’’ olarak rapor edildi. Sonuç: Dieulafoy lezyonu, nadir bir antitedir ve nedeni bilinmeyen gastrointestinal kanamalarda akla getirilmesi gerekir.

A Dieulafoy lesion presenting with massive gastric bleeding (case report)

Aim: Dieulafoy lesion is a rare, but potentially life-threatining cause of gastrointestinal bleeding. This vascular abnormality usually occurs in the proximal stomach but has been found in the esophagus, small intestine, and colon. Case Report: The patient underwent upper endoscopy for the evaluation of recurrent hematemesis of about 1 week duration. Endoscopy showed an acute bleeding artery, located on the posterior wall of the proximal stomach and sclerotherapy was performed. Because of the recurrence of bleeding, gastrotomy + wedge resection and primary suture was performed. Histopathological examination of the resection specimen was reported as Dieulafoy lesion. Result: Dieulafoy lesion is a rare entity and should be thought in the acute upper digestive tract bleeding with unknown etiology.
