Malign paranazal sinüs tümörlerinin tanısında bilgisayarlı tomografi ve manyetik rezonans görüntüleme

Amaç: Bu çalışmada malign paranazal sinüs tümörlerinin tanısında bilgisayarlı tomografi ve manyetik rezonans görüntülemenin etkinliğinin araştırılması amaçlandı. Yöntem: Ocak 2000-Mart 2004 tarihleri arasında klinik olarak paranazal sinüs tümörü düşünülen 20 olgu incelendi. Olguların tümüne bilgisayarlı tomografi ve manyetik rezonans görüntüleme incelemeleri yapıldı. Bilgisayarlı tomografi ve manyetik rezonans görüntülemede lezyonların iç yapı karakteristikleri, yerleşim yerleri ve çevre dokularla olan ilişkileri değerlendirildi. Bulgular: Çalışmaya alınan 20 olgudan 10’u erkek, 10’u kadındı. Yerleşim yerleri dağılımlarına göre; maksiller sinüste 11, etmoid sinüste 3, nazal kavitede 6 lezyon vardı. Bilgisayarlı tomografide tüm olgularda kemik yapılarda destrüksiyon izlendi. Osteosarkomda ve kondrosarkomda amorf kalsifikasyonlar vardı. Çevresel invazyon açısından bilgisayarlı tomografi ve manyetik rezonans görüntüleme arasında istatistiksel olarak anlamlı fark bulunmadı. Sonuç: Bilgisayarlı tomografi, kemik destrüksiyonlarını ve tümöral kalsifikasyonları değerlendirmede tercih edilmelidir. İstatistiksel olarak anlamlı fark olmasa da manyetik rezonans görüntüleme çevre dokulara olan invazyonu göstermede bilgisayarlı tomografiden daha üstündür. Bu yüzden paranazal sinüslerin malign tümörlerini değerlendirmede bilgisayarlı tomografi ve manyetik rezonans görüntüleme birlikte kullanılmalıdır.

Computed tomography and magnetic resonance imaging in diagnosis of malign tumors of the paranasal sinuses

Purpose: In this study, the effect of computed tomography and magnetic resonance imaging was researched in diagnosis of malign tumors of the paranasal sinuses. Methods: Between January 2000-March 2004, 20 cases that was thought paranasal sinus tumors as clinically were examined. Computed tomography and magnetic resonance imaging; the inner structure characteristics, the locations and the relations with the vicinity tissues of lesions were evaluated. Results: 10 men and 10 women were included into the study. According to the locations; there were 11 lesions in maxiller sinus, 3 lesions in ethmoid sinus, 6 lesions in nasal cavity. In all the cases destructions was found in bone structures on computed tomography. There were amorf calcifications in osteosarcoma and chondrosarcoma. For surrounding invasion, no statistically significant difference was found between computed tomography and magnetic resonance imaging. Conclusion: Computed tomography should be prefered in evaluating bone destructions and tumoral calcifications. Though there is no statistically significant difference, magnetic resonance imaging is superior to computed tomography in demonstrating surrounding tissue invasion. Therefore computed tomography and magnetic resonance imaging should be used together in evaluating malign tumors of the paranasal sinuses.
