Germ hücreli tümörlere eşlik eden intratübüler germ hücre neoplazisi

İntratübüler germ hücre neoplazisi, invaziv germ hücreli tümörlerle birlikteliğinden dolayı, testiküler germ hücreli malignensilerin preinvaziv dönemi olarak kabul edilir. Bu çalışmada, 158 testiküler germ hücreli tümörde, intratübüler germ hücreli neoplazi sıklığı, p53 ve plasental alkalen fosfataz immunohistokimyasal boyamaları, periodik-asid Schiff - diastaz ve arjirofilik nükleoler organize bölge histokimyasal boyamaları ile değerlendirildi. 158 testiküler germ hücreli tümörün 69’ unda, invaziv neoplastik odakların periferinde rezidüel seminifer tübüller bulundu. 65 olguda histomorfolojik olarak intratübüler germ hücre neoplazisi görüldü. 65 olgunun 63’ ünde (%96.9) , neoplastik intratübüler germ hücreleri plasental alkalen fosfataz ve periodik-asid Schiff (PAS) - diastaz ile glikojen lehine boyanma gösterdi. 48 (%73.8) olguda, atipik germ hücrelerinde p53 ile boyanma görüldü. Intratubular germ cell neplasia accompanying germ cell tumors

Intratubular germ cell neoplasia is considered a preinvasive stage of testicular germ cell malignancy because of its association with invasive germ cell tumors. In this study, the frequency of intratubular germ cell neoplasia in testicular germ cell tumors and immunohistochemical stains for p53, placental alkaline phosphatase and histochemical stains for periodic-acid Schiff (PAS)- diastase and argyrophilic nucleolar organizer regions were evaluated. 69 of 158 testicular germ cell tumors, residual seminiferous tubules were found at the periphery of the invasive neoplastic foci. Intratubular germ cell neoplasia was detected histomorphologicaly in 65 cases. Neoplastic intratubuler germ cells were reactive with placental alkaline phosphatase and positive for glicogene with periodic-acid Schiff - diastase stain in 63 (%96.9) of these cases. In 48 cases (%69), atypical germ cells were rective with p53. Argyrophilic nucleolar organizer regions count for the 63 cases which have intratubuler germ cell neoplasia was 5.83. It is concluded that intratubular germ cell neoplasia is present in most invasive germ cell tumors and the periodic-acid Schiff - diastase and placental alkaline phosphatase stains are useful in detecting intratubuler germ cell neoplasia.
