Abdusens paralizisine bağlı abdüksiyon kısıtlılığında vertikal rektus kaslarının tam transpozisyonunun etkinliği
Amaç: Çalışmamızda amacımız anormal baş pozisyonu oluşturan, abdüsens paralizisinin tedavisinde uyguladığımız vertikal rektusların tam tendon transpozisyonunun etkinliğini değerlendirmektir. Metod: Abdüsens paralizisine bağlı abdüksiyon kısıtlılığı görülen, beş bayan, dört erkek toplam dokuz olgunun dokuz gözünde yapılan fiksasyon sütürlü ve sütürsüz tam tendon transpozisyonlarının ve ilave olarak yapılan geriletmelerin etkinliği retrospektif olarak araştırıldı. Bulgular: Olguların yaş ortalaması 46.3, ortalama takip süresi 7 ay idi. Ameliyatlar öncesi ortalama 62PD olan kayma açısı, ameliyatlardan sonra ortalama 12PD'ye geriledi. Altı olguda (%67) baş pozisyonlarında düzelme, tüm olgularda binoküler görme alanında artmalar tespit edildi. Sonuç: Ciddi kozmetik problem oluşturan abdüsens paralizisine bağlı geniş açılı esotropyalı abdüksiyon kısıtlılıklarında, özellikle posterior fiksasyon sütürlü (Foster Op.) vertikal rektusların tam transpozisyonunun uygun geriletmeler eşliğinde son derece yüz güldürücü sonuçlar verdiği gözlendi.
Effectiveness of fully transpositions of vertical rectus muscles at restrictive abduction due to abducens palsy
Objective: The aim of this study is to evaluate the effectiveness of the transpositions of the vertical recti in abducens palsy that demonstrates abnormal head position. Method: In the 9 eyes of 9 cases (4 women, 5 men)' that has restricted abduction due to abducens palsy; the effectiveness of full tendon transpositions with or without fixation sutures and additional recessions has been evaluated retrospectively. Results: The median age of cases was 46.3 and median follow up time was 7 months. The mean deviation angle was 62 PD before operations, and it became 12 PD after operations. In 6 cases (67%), the head positions, and in all cases binocular visual field has improved. Conclusion: In restrictive abductions with large angle esotropia which cause serious cosmetic problems, full rectus transpositions especially with,posterior fixation suture (Foster operation) and with appropriate recessions produced satisfactory results.
