Karotis arter endarterektomisi ve anestezi yönetimi: 2 olgu sunumu
Karotis Arter Endarterektomisi (KAE) serebrovasküler inmenin (stroke) önlenmesi için etkin bir cerrahi yöntemdir. Karotis Arter Endarterektomisi ameliyatları anestezi uygulamalarını özellikli kılar. Birçok olguda kardiyovasküler ve nörolojik komplikasyonlar mevcuttur. Uygulanacak anestezi tekniği cerrahın deneyimine, anestezistin becerisine ve anestetik ekipmana göre seçilmelidir. Bu makalede, bilateral karotid arteriyel oklüzyonu olan ve bu nedenle 'karotis arter endarterektomisi yapılan ila olguda, ilk anestezi deneyimlerimizi sunduk. Bu iki olgunun da vertebro-baziller sistemden posterior kommunikan arterler aracılığıyla kollateral dolaşımı yeterli değildi. Bu nedenle, olgulara öncelikle Nöro-şirurji ekibi tarafından, serebral perfüzyonu korumak amacıyla orta serebral arter-superfisiyal temporal arter (STA-MCA) anastomozu yapıldı ve on gün sonra ikinci seansta tek taraflı karotis arter endarterektomisi uygulandı.
Carotid endarterectomy and anesthesia management: Two cases
Carotid endarterectomy is an efficient surgical method to prevent the stroke. Carotid endarterectomy operations require special anesthesia techniques. In many cases, there are cardiac and neurologic complications. Selection of the anesthesic technique depends on the experience of the surgeon, the skill of the anesthesiologist and the anesthesiologic equipments available. In this article, we present our first anestetic experience in two cases of bilateral carotid arterial occlusion that underwent carotid endarterectomy. In both cases, the collateral circulation by the means of posterior comminicating artery from the vertebral-basilar system was not sufficient. To provide the cerebral perfusion, anastomoses was performed between the middle cerebral artery and the superficial tempducted.
